摘 要:目的:通过Meta分析对瑞马唑仑和丙泊酚用于气管插管全身麻醉的安全性和有效性进行比较。方法:系统检索Pubmed、Web of science、Embase、中国知网、维普、万方和中国生物医学全文数据库从建库至2022年8月有关瑞马唑仑和丙泊酚用于气管插管全身麻醉的临床随机对照试验(RCT),并查询纳入文献所引用的参考文献。对所有符合纳入标准的文献进行质量评价和数据提取,采用RevMan5.4软件进行Meta分析。结果:共纳入16个RCT,1895例患者。Meta分析显示:与丙泊酚相比,瑞马唑仑能明显降低低血压的发生率(OR=0.31,95%CI 0.24~0.40),降低低氧血症的发生率(OR=0.16,95%CI 0.06~0.48),降低心动过缓的发生率(OR=0.13,95%CI 0.05~0.35),降低注射痛的发生率(OR=0.01,95%CI 0.00~0.03),并且可以缩短术后拔管时间(MD=-49.47 min,95%CI-73.95~-24.99 min),但瑞马唑仑麻醉诱导后入睡时间明显延长(MD=27.30 s,95%CI 22.69~33.91 s)。结论:在气管插管全身麻醉中,与丙泊酚相比,瑞马唑仑引起低血压、低氧血症等不良事件的概率更低,术后拔管时间更短,是一种可用于气管插管全身麻醉的安全有效的药物。Objective:To compare the safety and efficacy of remazolam and propofol for tracheal intubation general anesthesia through metaanalysis.Methods:Randomized controlled clinical trials(RCTS)on remazolam and propofol for general anesthesia with endotractal catheterization were systematically searched from Pubmed,Web of science,Embase,CNKI,VIP,Wanfang and China Biomedical full-text database from establishment to August 2022,and references cited in the included literatures were searched.Quality evaluation and data extraction were performed for all references that met the inclusion criteria,and meta-analysis was performed using RevMan5.4 software.Results:A total of 16 RCTS with 1895 patients were included.Meta-analysis showed that:Compared with propofol,remazolam significantly reduced the incidence of hypotension(OR=0.31,95%CI 0.24-0.40),hypoxemia(OR=0.16,95%CI 0.06-0.48)and bradycardia(OR=0.13,95%CI 0.06-0.48.95%CI 0.05~0.35),decreased the incidence of injection pain(OR=0.01,95%CI 0.00~0.03),and shortened the postoperative extubation time(MD=-49.47 min,95%CI-73.95~-24.99 min).However,the sleep time after remazolam induction was significantly prolonged(MD=27.30 s,95%CI 22.69~33.91 s).Conclusion:Compared with propofol,Remazolam has a lower probability of causing hypotension,hypoxemia and other adverse events,and the postoperative extubation time is shorter.Remazolam is a safe and effective drug for endotracheal intubation general anesthesia.
