黄芪赤风汤加味治疗良性阵发性位置性眩晕(气虚血瘀证)复位后残余头晕的疗效

摘　　要：目的 观察黄芪赤风汤加味治疗良性阵发性位置性眩晕(BPPV)(气虚血瘀证)复位后残余头晕疗效及对脑血流速度、血液流变学指标的影响,并探讨其作用机制。方法 80例患者随机分为治疗组与对照组各40例,对照组给予倍他司汀治疗,治疗组给予黄芪赤风汤加味治疗。两组均治疗2周。比较两组治疗前后前庭症状指数(VSI)评分、Berg平衡量表(BBS)评分、焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分、血液流变学指标(全血高切黏度、全血低切黏度、血浆黏度)及脑血流速度(左侧椎动脉、右侧椎动脉、基底动脉血流峰值)水平。结果 两组治疗后VSI、SAS、SDS评分与治疗前比较均降低,而BBS评分则升高(均P <0.05),且治疗组VSI、SAS、SDS评分均低于对照组,BBS评分则高于对照组(均P <0.05)。治疗后两组高切全血黏度、低切全血黏度、血浆黏度方面均较治疗前降低(均P <0.05),且治疗组均低于对照组(P <0.05)。治疗后两组BA、LVA、RVA的血流峰值与治疗前比较均改善(P <0.05);且治疗组均优于对照组(P <0.05)。结论 黄芪赤风汤加味治疗良性阵发性位置性眩晕(气虚血瘀证)复位后残余头晕可显著缓解头晕症状,减轻焦虑抑郁情绪,改善脑血流速度及血液黏稠度。Objective:To investigate the efficacy of Modified Huangqi Chifeng Decoction in the treatment of re-sidual dizziness afer reduction of benign paroxysmal positional vertigo(BPPV)(blood stasis due to qi deficiency type)and its and effects on cerebral blood flow velocity and blood rheology,and to explore its mechanism.Meth-ods:80 patients were randomly divided into the treatment group and control group,40 cases in each group.The control group was treated with betahistine,and the treatment group was given Modified Huangqi Chifeng Decoction.Both groups were treated for 2 weeks.The vestibular symptom index(VSI)score,Berg balance scale(BBS)score,self-rating anxiety scale(SAS)score,self-rating depression scale(SDS)score,blood rheological indexes(whole blood high cut viscosity,whole blood low cut viscosity,plasma viscosity)and cerebral blood flow velocity(left verte-bral artery,right vertebral artery,basilar artery peak blood flow)before and after treatment were compared.Re-sults:Compared with before treatment,the VSI,SAS and SDS scores of the two groups were reduced,while the BBS scores were increased(all P<0.05),and the VSI,SAS and SDS scores in the treatment group were lower than those in the control group,and the BBS scores were higher than those in the control group(all P<0.05).After treatment,the viscosity of high-cut whole blood,low-cut whole blood viscosity and plasma viscosity were all lower than those before treatment(all P<0.05),and the indexes in the treatment group were lower than those in the con-trol group(P<0.05).The peak blood flow of BA,LVA and RVA in both groups after treatment was improved com-pared with that before treatment(P<0.05),and the indexes in the treatment group were beter than those in the control group(P<0.05).Conclusion:Modified Huangqi Chifeng Decoction for residual dizziness after reduction of benign paroxysmal positional vertigo(blood stasis due to qi deficiency type)can significantly relieve dizziness symptoms,reduce anxiety and depression,and improve cerebral blood flow velocity and bl
