激光包皮环切术中系带延长的三种简便易行的手术方法

摘　　要：目的:探讨和介绍激光包皮环切术中系带延长的三种简便易行的手术方法。方法:研究对象为2014年1月至2019年12月间行激光包皮环切术与包皮系带延长术的患者206例,其中系带横断然后纵行间断缝合切口延长126例,纵行间断缝合系带下方两侧包皮内板延长46例,纵行间断缝合系带下方两侧包皮内板延长,阴茎背侧正中包皮内板纵行切开,修剪切口两直角,延长包皮内板周径34例,观察并记录患者的手术效果。结果:所有患者均在20~40 min内完成手术,术中出血少,切缘整齐,术后恢复好,外形美观,阴茎勃起良好,龟头无下屈、无牵扯不适。结论:激光包皮环切术具有手术速度快、出血少、切缘整齐等优点,但同时如何行系带延长的研究缺乏,笔者应用以上三种包皮系带延长术(其中第二、第三两种术式为新方法)延长包皮系带简单可靠、效果良好。Objective:To discuss and introduce three simple and feasible surgical methods for frenulum extension in laser circumcision.How:The subjects of this study were 206 patients who underwent laser circumcision and frenulum extension between January 2014 and December 2019.Among them,126 patients underwent transverse frenulum rupture followed by longitudinal intermittent suture to extend the incision,46 patients underwent longitudinal intermittent suture to extend both sides of the inner plate below the frenulum,and 46 patients underwent longitudinal intermittent suture to extend both sides of the inner plate below the frenulum.34 cases were incised longitudinally on the dorsal median prepuce plate of the penis.Two right angles were pruned and the circumference of the prepuce plate was lengthened.The results were observed and recorded.Results:All the patients completed the operation within 20~40 min,less bleeding during the operation,the incisal margin was neat,the postoperative recovery was good,the appearance was beautiful,the penis erection was good,the glans head was not bent down,no discomfort.Conclusion:Laser circumcision has the advantages of fast operation,less bleeding and regular incisal margins,but there is a lack of research on how to extend the frenulum.The author applied the above three frenulum lengthening procedures(the second and third are new methods)to extend the frenulum of the foreskin simply,reliably and effectively.
