无创呼吸机治疗慢阻肺合并重症呼吸衰竭研究进展

摘　　要：慢性阻塞性肺疾病(简称,慢阻肺)是临床常见呼吸系统疾病之一,主要病因是呼吸功能障碍引起气流受限并造成的具有不可逆性特征的病理改变,病情通常比较危重,还可导致重症呼吸衰竭等严重并发症,不仅会影响患者的健康,甚至会危及其生命安全。近年来环境污染问题不断加剧导致慢阻肺合并重症呼吸衰竭患者数量明显增加,长期吸烟、空气污染、职业性粉尘、感染及化学物质等均为该病的常见诱因,需要及时为患者提供有效的治疗干预以改善病情及患者生活质量。随着医学技术的快速发展,在该疾病的治疗中机械通气技术的应用逐渐增加,机械通气可大大提升患者生存率,但常规有创机械通气创伤性较大,并发症风险高,因此疗效欠佳。而无创呼吸机属于无创机械通气技术,无需建立人工气道即可为呼吸困难者通过口鼻罩、面罩或鼻罩等途径并连接呼吸机进行正压辅助通气,该种方法在很多呼吸衰竭患者的治疗中均获得了良好的疗效。文章将针对无创呼吸机在慢阻肺合并重症呼吸衰竭患者治疗中的应用进展进行分析,旨在为该类患者的机械通气治疗提供参考。Chronic obstructive pulmonary disease(COPD)is one of clinical common respiratory system disease,is the main cause of respiratory dysfunction caused by limited airflow and cause has irreversibility of pathological change,the illness is usually critically ill,still can cause serious complications,such as severe respiratory failure will not only affect the health of the patient,and even endanger the safety of life.In recent years,the increasing environmental pollution has led to a significant increase in the number of patients with COPD complicated with severe respiratory failure.Long-term smoking,air pollution,occupational dust,infection and chemical substances are common causes of the disease,and it is necessary to provide effective treatment and intervention for patients in a timely manner to improve their condition and quality of life.With the rapid development of medical technology,the application of mechanical ventilation technology has gradually increased in the treatment of this disease.Mechanical ventilation can greatly improve the survival rate of patients,but conventional invasive mechanical ventilation is more traumatic and has a high risk of complications,so the efficacy is not good.However,non-invasive ventilator is a noninvasive mechanical ventilation technology,which can provide positive pressure auxiliary ventilation for patients with respiratory distress through mouth and nose mask,mask or nose mask and ventilator without establishing artificial airway.This method has achieved good efficacy in the treatment of many patients with respiratory failure.This paper will analyze the application progress of non-invasive ventilator in the treatment of copd patients complicated with severe respiratory failure,in order to provide reference for the treatment of mechanical ventilation in these patients.
