摘 要:目的:分析对比鼻咽癌颅底骨侵犯诊断中磁共振弥散加权成像(DWI)与计算机体层成像(CT)扫描的应用价值。方法:选取2022年1月~2023年1月本院收治鼻咽癌患者65例,治疗前全部患者均接受磁共振检查及CT检查,对比分析检查结果。结果:全部65例患者中,CT检查结果显示21例患者的颅底骨质完全正常,19例患者颅底骨质溶性破坏,边界模糊,边缘硬化,骨质结构丧失,破坏灶内无钙化,其中7例患者肿物对一侧卵圆孔造成侵犯,卵圆孔周边骨质坏,边缘模糊,孔腔扩大。磁共振扫描检查结果发现38例患者存在颅底骨质侵犯,对枕骨基底部、斜坡、双侧或一侧蝶骨大翼、蝶骨体、岩尖造成累及。CT检查的阳性检出率为29.23%(19/65),敏感性为50.0%(19/38);磁共振检查的阳性检出率为58.46%(38/65),敏感性为100.0%(38/38);两种检查方法的特异性均为100.0%;磁共振检查的阳性检出率、敏感性均显著高于CT检查(P<0.05)。结论:与CT检查相比较,DWI诊断鼻咽癌颅底骨侵犯的准确性更高,能为鼻咽癌的疾病分期及治疗方案的制定提供指导。Objective:To analyze and compare the application value of magnetic resonance diffusion weighted imaging(DWI)and computed tomography(CT)scanning in diagnosis of skull base invasion in nasopharyngeal carcinoma.Methods:65 patients with nasopharyngeal carcinoma were selected,all of whom were admitted to our hospital from January 2022 to January 2023.Before treatment,all patients underwent magnetic resonance examination and CT examination,and the examination results were compared and analyzed.Results:Among all 65 patients,CT examination results showed that the skull base bone was completely normal in 21 patients,and the skull base bone was dissolved in 19 patients with blurred boundary,hardening of the edge,loss of bone structure,and no calcification in the lesion.Among them,7 patients had mass invasion of one side of the foramen ovale,bad bone around the foramen ovale,blurred edge,and enlarged cavity.Magnetic resonance imaging(MRI)scans revealed 38 patients with invasions of skull base,involving the occipital base,slopes,bilateral or unilateral sphenoid wings,sphenoid bodies,and rock tips.The positive rate and sensitivity of CT were 29.23%(19/65)and 50.0%(19/38)respectively.The positive detection rate and sensitivity of MRI were 58.46%(38/65)and 100.0%(38/38).The specificity of the two methods was 100.0%.The positive detection rate and sensitivity of MRI were significantly higher than those of CT(P<0.05).Conclusion:In the diagnosis of nasopharyngeal carcinoma skull base invasion,compared with computerized tomography,magnetic resonance diffusion weighted imaging has higher diagnostic accuracy,and can provide guidance for the development of disease staging and treatment of nasopharyngeal carcinoma.
