彩超在小乳腺癌与乳腺增生鉴别诊断中的临床应用

摘　　要：目的:分析小乳腺癌、乳腺增生疾病特点,评价彩色多普勒超声的鉴别诊断价值。方法:选择2019年9月~2020年5月本院收治的小乳腺癌疾病患者50例、乳腺增生疾病患者50例,经伦理委员会审核批准,手术病理证实。全部患者均行彩超诊断,比较2组受检患者的多普勒图像、收缩期峰值流速(PSV)、血流阻力指数(RI)以及血流分级情况。结果:多普勒图像方面,小乳腺癌患者病灶呈不规则、边界粗糙、不均匀回声,与乳腺增生疾病患者比较,P<0.05;SPV、RI值方面,小乳腺癌患者的SPV值(21.35±4.50)cm/s以及RI值(1.08±0.35)均明显高于乳腺增生患者,P<0.05;血流分级方面,小乳腺癌患者血流分级集中在Ⅱ级、Ⅲ级,乳腺增生患者血流分级集中在0级、Ⅰ级,差异显著,P<0.05。结论:彩色多普勒超声征象能够准确鉴别小乳腺癌、乳腺增生,有助于疾病的早期治疗,促进患者预后,是有效诊断方式。Objective:To analyze the characteristics of small breast cancer and breast hyperplasia,and to evaluate the value of color Doppler ultrasound in the differential diagnosis.Methods:Select patients with small breast cancer disease(n=50)and breast hyperplasia disease(n=50)who were admitted to our hospital from September 2019 to May 2020.The patients were reviewed and approved by the ethics committee and confirmed by surgical pathology.All patients were diagnosed by color Doppler ultrasound,and the Doppler images,peak systolic velocity(PSV),resistance index(RI)and blood flow classification of the two groups of patients were compared.Results:In terms of Doppler image,the focus of small breast cancer patients was irregular,rough border,and uneven echo,compared with breast hyperplasia patients,P<0.05;In terms of SPV and RI,the SPV(21.35±4.50)cm/s and RI(1.08±0.35)of small breast cancer patients were significantly higher than those of breast hyperplasia patients(P<0.05);In terms of blood flow grading,the blood flow grading of small breast cancer patients was concentrated in grade Ⅱ and grade Ⅲ,and that of breast hyperplasia patients was concentrated in grade 0 and grade Ⅰ,with significant difference(P<0.05).Conclusion:Color Doppler ultrasound can accurately identify small breast cancer and breast hyperplasia,help early treatment of the disease,and promote the prognosis of patients.It is an effective diagnostic method.
