摘  要:目的:观察CT平扫+CTA(含多期相)+CTP一站式扫描对急性缺血性脑卒中脑组织血流灌注、脑侧支血流循环状态及责任血管狭窄的评估价值分析。方法:选择2022年1月~2022年11月本院收治的130例急性缺血性脑卒中患者,接受CT平扫+CTA(含多期相)+CTP一站式扫描。分析CTPI阳性患侧核心梗死区、缺血半暗带与健侧相应区域CTPI参数,侧支循环良好与不良者核心梗死区与健侧CTPI参数,责任血管不同狭窄程度患者CTPI血流灌注情况。结果:CTPI结果显示,117例患者的脑组织血流灌注发生异常,阳性率为90.00%(117/130)。早期急性卒中分级评估侧支循环良好者43例,侧支循环不良者87例。轻度27例,中度67例,重度或闭塞36例。CTPI阳性患侧核心梗死区及缺血半暗带脑血流量值、脑血容量值均小于健侧,平均通过时间、达峰时间均长于健侧(P<0.05)。侧支循环良好者患侧脑血流量值小于健侧,且平均通过时间及达峰时间均长于健侧,侧支循环不良者患侧脑血流量值及脑血容量值均小于健侧,且平均通过时间及达峰时间均长于健侧(P<0.05),血流灌注异常责任血管重度狭窄或闭塞发生率高于正常者(P<0.05)。结论:在急性缺血性脑卒中患者中,以CT平扫+CTA(含多期相)+CTP一站式扫描,可早期明确脑组织血流灌注情况、脑侧支血流循环状态及责任血管狭窄程度,判断缺血半暗带及核心梗死区,为临床治疗提供依据。Objective:To observe the evaluation value of CT plain scanning+CTA(including multi-phase)+CTP one-stop scanning on cerebral tissue blood flow perfusion,cerebral collateral blood flow circulation and responsible vessel stenosis in patients with acute ischemic stroke.Methods:130 patients with acute ischemic stroke were treated in our hospital(January 2022 to November 2022),and received CT plain scan+CTA(including multiphase)+CTP one-stop scan.CTPI parameters in the core infarction area,ischemic penumbra and corresponding areas on the healthy side of patients with positive CTPI were analyzed.CTPI parameters in the core infarction area and the healthy side of patients with good or bad collateral circulation were analyzed.Results:Under the results of CTPI,117 patients had abnormal cerebral blood perfusion,and the positive rate was 90.00%(117/130).43 patients had good collateral circulation and 87 patients had poor collateral circulation.There were 27 cases of mild stenosis,67 cases of moderate stenosis,and 36 cases of severe stenosis or occlusion.The values of cerebral blood flow and cerebral blood volume in the core infarcted area and ischemic penumbra of CTPI positive patients were lower than those in the healthy side,and the average transit time and peak time were longer than those in the healthy side(P<0.05).In patients with good collateral circulation,the cerebral blood flow value of the affected side was lower than that of the healthy side,and the average transit time and peak time were longer than those of the healthy side.In patients with poor collateral circulation,the cerebral blood flow value and cerebral blood volume value of the affected side were lower than those of the healthy side,and the average transit time and peak time were longer than those of the healthy side(P<0.05),but there was no significant difference between the cerebral blood volume values of the affected side and the healthy side(P>0.05).The incidence of severe stenosis or occlusion of the responsible vessels with abnormal blood flow perf
