摘　　要：目的:探讨在ICU患者静脉输液中使用超声引导下留置静脉中线导管与PICC的应用效果分析。方法:选择2020年6月~2020年12月入住福建省某三甲医院SICU的患者40例,采用随机数字表法分为对照组和实验组各20例。对照组采用PICC进行静脉输液,实验组采用静脉中线导管进行静脉输液。比较两组置管成功前穿刺失败次数、穿刺置管时间、留置时间、常见并发症发生率(静脉炎、静脉渗漏、导管堵塞、穿刺点感染)、经济费用(置入费用和维护费用)。结果:实验组置管成功前穿刺失败次数(1.02±0.39)次、穿刺置管时间(11.32±1.98)min、留置时间(16.01±1.10)d低于对照组置管成功前穿刺失败次数(3.01±1.05)次、穿刺置管时间(20.14±2.07)min、留置时间(30.76±0.84)d(P<0.05);实验组常见并发症总发生率40.00%高于对照组的15.00%(P<0.05);经济费用(置入费用和维护费用)低于对照组(P<0.05)。结论:中等长度导管较PICC在ICU患者静脉输液中应用效果更佳,能够缩短置管时间,置管后待用时间,降低患者治疗费用,但并发症发生率较PICC患者高,提示应加强中长线导管的维护,减少相关并发症的发生。Objective:To explore the application effect of ultrasound-guided indwelling venous midline catheter and PICC in intravenous infusion of ICU patients.Methods:40 patients admitted to SICU in a third class hospital in Fujian Province from June 2020 to December 2020 were randomly divided into control group and experimental group.PICC was used for intravenous infusion in the control group and midline catheter was used for intravenous infusion in the experimental group.The puncture failure times,puncture time,indwelling time,the incidence of common complications(phlebitis,venous leakage,catheter blockage,puncture point infection)and economic expenses(implantation expenses and maintenance expenses)were compared between the two groups.Results:The number of puncture failures(1.02±0.39),puncture time(11.32±1.98)min and retention time(16.01±1.10)d in the experimental group were lower than those in the control group(3.01±1.05),puncture time(20.14±2.07)min and retention time(30.76±0.84)d(P<0.05);The total incidence of common complications was 40.00%higher than 15.00%in the control group(P<0.05);The economic cost(implantation cost and maintenance cost)was lower than that in the control group(P<0.05).Conclusion:The application effect of medium length catheter in ICU patients is better than that of PICC.It can shorten the catheterization time,the waiting time after catheterization and reduce the treatment cost of patients,but the incidence of complications is higher than that of PICC patients,suggesting that the maintenance of medium and long-term catheter should be strengthened to reduce the incidence of related complications.
