纵隔型肺癌CT及MRI的临床表现及诊断效果分析

摘　　要：目的:研究分析纵膈型肺癌的MRI和CT临床表现,并且比较MRI和CT诊断纵膈型肺癌的临床价值。方法:选择2017年1月~2023年1月收治的113例纵膈型肺癌患者为研究对象,术前均行MRI和CT检查,将手术病理结果作为金标准,比较分析两种检查方法的诊断结果和临床表现。结果:所有患者均顺利完成检查,本组113例患者中,65例为周围型肺癌,48例为中央型肺癌;MRI和CT诊断纵膈型肺癌的准确率分别为94.69%(107/113)、92.93%(105/113),对比无统计学意义(P>0.05);同时,在CT检查中,可见病变肿块具有不规则的边缘、呈毛刺状,纵膈淋巴结异常肿大,而MRI的影像学特征与CT检查基本一致,并且二者均能清晰显示病变肿块入侵胸膜、胸壁以及胸椎情况。结论:在纵膈型肺癌的临床诊断中,MRI和CT检查均具有较高的准确率,必要的情况下,联合运用二者,能够为制定针对性治疗方案和判断患者预后提供一定的参考依据。Objective:To study and analyze the clinical manifestations of MRI and CT in the diagnosis of mediastinal lung cancer,and to compare the clinical value of MRI and CT in the diagnosis of mediastinal lung cancer.Methods:A total of 113 patients with mediastinal lung cancer admitted from January 2017 to January 2023 were selected as subjects.MRI and CT examinations were performed before surgery,and pathological results were used as the gold standard to compare the diagnostic results and clinical manifestations of the two methods.Results:All patients completed the examination successfully.Among the 113 patients in this group,65 cases were peripheral lung cancer and 48 cases were central lung cancer.The accuracy rates of MRI and CT in diagnosing mediastinal lung cancer were 94.69%(107/113)and 92.93%(105/113),respectively,with no statistical significance(P>0.05).At the same time,CT examination showed that the pathological mass had irregular edges,was burr like,and abnormal enlargement of mediastinal lymph nodes.MRI imaging features were basically consistent with CT examination,and both of them could clearly show the pathological mass invading pleura,chest wall and thoracic vertebra.Conclusion:In the clinical diagnosis of mediastinal lung cancer,both MRI and CT have high accuracy.If necessary,the combination of MRI and CT can provide certain reference for the formulation of targeted treatment plan and the judgment of patient prognosis.
