HIFU控制恶性肿瘤腹膜后淋巴结转移所致疼痛的疗效分析

摘　　要：目的:探究高强度聚焦超声(high intensity focus ultrasound,HIFU)是否有助于缓解恶性肿瘤腹膜后淋巴结转移所致疼痛的发生。方法:收集2020年4月~2021年4月本院收治的26例腹膜后淋巴结转移患者。参考随机数字表法平均分为两组(n=13),后两组分别各自实施常规放射治疗方案(对照组)以及HIFU联合放射治疗(观察组)。术后比较两组患者的疼痛评分、阻塞感改善情况以及生活质量评分等。结果:两组患者疼痛NRS分数呈现降低的趋势,观察组术后各时间点NRS评分均显著低于对照组,对比差异有显著统计学意义(P<0.05);观察组患者阻塞感的缓解程度优于对照组,对比差异有统计学意义(P<0.05);观察组患者生活质量显著优于对照组,对比差异有统计学意义(P<0.05)。两组患者在治疗过程中除去患处皮肤疼痛感这一常见副作用外,无皮肤烧伤、脏器穿孔、胰瘘、肿瘤破裂等不良事件。结论:HIFU作为一种安全有效的治疗方案值得临床进一步研究与推广。Objective:To investigate whether HIFU(high intensity focused ultrasound)is helpful to relieve pain caused by retroperitoneal lymph node metastasis of malignant tumors.Methods:26 subjects with retroperitoneal lymph node metastasis admitted to our hospital from April 2020 to April 2021 were collected.The subjects were equally divided into two groups(n=13)according to random number table.The latter two groups implement different treatment regimens.One is given conventional radiotherapy regimen(control group)and another is given HIFU combined with radiotherapy(research group),respectively.After the operation,we collected and organized the pain score,improvement of obstruction sensation and quality of life score,and compared between the above two groups.Results:The pain NRS scores of the two groups of patients showed a decreasing trend,and the NRS scores of the observation group were significantly lower than those of the control group at all time points after surgery,with a statistically significant difference(P<0.05);The relief of obstructive sensation in the observation group was better than that in the control group,and the difference was statistically significant(P<0.05);The quality of life of the observation group patients was significantly better than that of the control group,and the difference was statistically significant(P<0.05).During the treatment process,except for the common side effect of skin pain in the affected area,there were no adverse events such as skin burns,organ perforation,pancreatic fistula,or tumor rupture in the two groups of patients.Conclusion:HIFU as a safe and effective treatment is worthy of further clinical research and promotion.
