摘　　要：目的:分析ICU急性心力衰竭合并呼吸衰竭治疗中无创呼吸机的应用效果。方法:选择2019年11月~2021年11月本院重症医学科ICU收治的62例ICU急性心力衰竭合并呼吸衰竭患者,依据治疗环节所用措施方法差异,等分为人数相同的两组,给予对照组(n=31例)对症支持治疗,实验组(n=31例)给予无创呼吸机治疗,记录两组的抢救成功率和血气指标及其心功能改善指标情况。结果:实验组抢救成功率96.77%(30/31)明显高于对照组抢救成功率74.19%(23/31),差异有统计学意义(P<0.05);实验组PaCO_(2)、症状改善时间、呼吸节律积分值均小于对照组,差异有统计学意义(P<0.05);实验组患者在左心室射血分数(LVEF)值、左室舒张末期容积指数(LVEDVI)、室间隔厚度(IVST)与对照组对比差异显著,有统计学意义(P<0.05)。结论:ICU急性心力衰竭合并呼吸衰竭患者的治疗中无创呼吸机治疗的价值突出,临床有效性和安全性效果较高,并可改善患者预后,可在后续患者的临床治疗实践中加以应用。Objective:To analyze the effect of noninvasive ventilator in the treatment of acute heart failure complicated with respiratory failure in ICU.Methods:62 patients with acute heart failure complicated with respiratory failure in ICU of the department of critical medicine of our hospital from November 2019 to November 2021 were selected.According to the difference of nursing measures,they were equally divided into two groups with the same number.The control group(n=31 cases)was given symptomatic support treatment,and the experimental group(n=31 cases)was given noninvasive ventilator treatment,the rescue success rate,blood gas index and improvement index of cardiac function of the two groups were compared.Results:The rescue success rate of 96.77%(30/31)in the experimental group was significantly higher than that of 74.19%(23/31)in the control group,with a statistically significant difference(P<0.05);The experimental group had significantly lower PaCO_(2),symptom improvement time,and respiratory rhythm integral values than the control group(P<0.05);The left ventricular Ejection fraction(LVEF),left ventricular end diastolic volume index(LVEDVI),and ventricular septal thickness(IVST)in the experimental group were significantly different from those in the control group(P<0.05).Conclusion:In the treatment of patients with acute heart failure complicated with respiratory failure in ICU,noninvasive ventilator therapy has prominent value,high clinical efficacy and safety,and can improve the prognosis of patients,which can be applied in the clinical treatment practice of follow-up patients.
