摘　　要：目的探讨影响慢性阻塞性肺疾病(COPD)患者吸入噻托溴铵粉雾剂依从性因素及预防措施。方法对117例COPD患者吸入噻托溴铵粉雾剂依从性进行评价,比较不同依从性COPD患者相关因素上的差异。结果117例COPD患者中,依从性好82例(70.09%),依从性差35例(29.91%)。吸入噻托溴铵粉雾剂依从性差患者年龄≥75岁、小学以下文化、抑郁自评量表(SDS)评分≥50、未婚、COPD病程≥15年、[健康指数(体重指数、梗阻、呼吸困难、运动能力)](BODE指数)≥5、未随访、自费、经济较差比例高于依从性好组(P<0.05),Logistic回归分析影响吸入噻托溴铵粉雾剂依从性因素为:小学以下文化(OR=6.414,95%CI:1.014~21.654)、SDS评分≥50(OR=5.923,95%CI:1.125~23.562)、年龄≥75岁(OR=4.543,95%CI:1.643~12.765)、经济较差(OR=3.910,95%CI:2.018~13.675)、COPD病程≥15年(OR=3.246,95%CI:1.976~16.227)。结论COPD患者吸入噻托溴铵粉雾剂依从性较低,影响因素复杂。临床应实施相应措施提高吸入噻托溴铵粉雾剂依从性。Objective To explore the factors affecting the compliance of COPD inhaled tiotropium bromide powder sprayand preventive measures.Methods The compliance of 117 COPD patients with inhaled tiotropium bromide powder spray was evaluated,compare the differences in related factors of COPD patients with different compliance.Results Among 117 patients with COPD,82(70.09%)patients had good compliance,35(29.91%)patients had poor compliance.The proportion of patients with poor compliance to inhaled tiotropium bromide powder spray was higher than that of the good compliance group(P<0.05),including age≥75 years,education below primary school,SDS score≥50,unmarried,COPD course≥15 years,BODE index≥5,no follow-up,self funded,and financially poor.Logistic regression analysis the factors affecting the compliance of inhaled tiotropium bromide powder spray:education below primary school(OR=6.414,95%CI:1.014~21.654),SDS score≥50(OR=5.923,95%CI:1.125~23.562),age≥75 years(OR=4.543,95%CI:1.643~12.765),financially poor(OR=3.910,95%CI:2.018~13.675),COPD course≥15 years(OR=3.246,95%CI:1.976~16.227).Conclusion COPD patients inhaled tiotropium bromide powder spray has low compliance and complex influencing factors.Corresponding measures should be implemented in clinical practice to improve the compliance of inhaled tiotropium bromide powder spray.
