ANA、抗ENA抗体联合抗ds-DNA抗体对自身免疫性疾病的诊断价值

摘　　要：目的 研究自身免疫性疾病诊断中抗核抗体(ANA)、抗可提取性核抗原抗体(抗ENA抗体)、抗双链DNA抗体(抗ds-DNA抗体)联合诊断的临床价值。方法 选取240例自身免疫性疾病患者为观察组,另选取240例健康体检者为对照组。两组研究对象均测定血清抗ENA抗体、ANA及抗ds-DNA抗体。对比两组血清ANA、抗ds-DNA抗体、抗ENA抗体阳性率,观察组不同疾病患者的抗ENA抗体阳性率。结果 观察组血清抗ds-DNA抗体阳性率60.42%、ANA阳性率82.08%、抗ENA抗体阳性率64.58%均显著高于对照组的0.42%、0.42%、0,差异具有统计学意义(P<0.05)。观察组不同疾病患者的抗nRNP/Sm抗体、抗Sm抗体、抗SS-A抗体、抗Ro-52抗体、抗SS-B抗体、抗SCL-70抗体、抗PM-SCL抗体、抗J0-1抗体阳性率对比,差异具有统计学意义(P<0.05)。结论 自身免疫性疾病中ANA、抗ENA抗体、抗ds-DNA抗体联合检测,可为临床诊治提供可靠依据。Objective To study the clinical value of anti-nuclear antibody(ANA)and anti-extractable nuclear antigen(anti-ENA)antibody combined with anti-double stranded DNA(anti-ds-DNA)antibody for the diagnosis of autoimmune diseases.Methods 240 patients with autoimmune diseases were selected as the observation group,and 240 healthy subjects were selected as the control group.Serum anti-ENA antibody,ANA and anti-ds-DNA antibody were measured in both groups.The positive rates of serum ANA,anti-ds-DNA antibody and anti-ENA antibody in the two groups were compared,as well as the positive rates of anti-ENA antibody in patients with different diseases in the observation group.Results The positive rates of serum anti-ds-DNA antibody,ANA and anti-ENA antibody were 60.42%,82.08%and 64.58%in the observation group,which were significantly higher than those of 0.42%,0.42%and 0 in the control group,and the differences were statistically significant(P<0.05).In the observation group,the positive rates of anti-nRNP/Sm antibody,anti-Sm antibody,anti-SS-A antibody,anti-Ro-52 antibody,anti-SS-B antibody,anti-SCL-70 antibody,anti-PM-SCL antibody,and anti-J0-1 antibody were compared in patients with different diseases,and the differences were statistically significant(P<0.05).Conclusion In autoimmune diseases,the combination of ANA,anti-ENA antibody and anti-ds-DNA antibody can provide a reliable basis for clinical diagnosis and treatment.
