多轴锁定钢板在胫骨平台骨折患者中的治疗效果及对肢体活动度的影响研究

摘　　要：目的探讨多轴锁定钢板在胫骨平台骨折患者中的治疗效果及对肢体活动度的影响。方法选择2018年6月—2022年6月北京积水潭医院贵州医院收治的胫骨平台骨折患者83例作为研究对象,依据随机抽签法分为对照组44例和研究组39例。对照组予以普通锁定钢板内固定,研究组予以多轴锁定钢板内固定。比较两组治疗效果,手术时间、术中出血量、住院时间、骨折愈合时间、完全负重时间,肢体活动度以及并发症发生率。结果研究组总治疗有效率为100.00%高于对照组的86.38%,差异有统计学意义(χ^(2)=3.880,P<0.05)。研究组手术时间、术中出血量、住院时间、骨折愈合时间、完全负重时间均低于对照组,差异有统计学意义(P<0.05)。治疗后,研究组TPA、PSA低于对照组,差异有统计学意义(P<0.05);研究组肢体稳定性、活动范围评分高于对照组,差异有统计学意义(P<0.05)。研究组总并发症发生率为5.13%,低于对照组的20.45%,差异有统计学意义(P<0.05)。结论相比普通锁定钢板内固定多轴锁定钢板在胫骨平台骨折患者中的治疗效果更为显著,可改善患者肢体活动度,缩短手术、住院及骨折愈合时间,减少并发症。Objective To investigate the therapeutic effect of multiaxial locking plate in patients with tibial plateau fractures and its influence on the range of motion of limbs.Methods A total of 83 patients with tibial plateau fractures admitted to Beijing Jishuitan Hospital Guizhou Hospital from June 2018 to June 2022 were selected as the research objects.They were randomly divided into a control group of 44 cases and a research group of 39 cases.The control group received internal fixation with common locking plates,and the study group received internal fixation with multiaxial locking plates.The treatment effect,operation time,intraoperative blood loss,hospitalization time,fracture healing time,full weight-bearing time,limb activity and complication rate were compared between the two groups.Results The total effective rate of the study group was 100.00%higher than that of the control group 86.38%,the difference was statistically significant(χ^(2)=3.880,P<0.05).The operation time,intraoperative blood loss,hospitalization time,fracture healing time,and full weight-bearing time in the study group were all lower than those in the control group,the difference was statistically significant(P<0.05).After treatment,TPA and PSA in the study group were lower than those in the control group,the difference was statistically significant(P<0.05);the study group's limb stability and range of motion scores were higher than those of the control group,the difference was statistically significant(P<0.05).The total incidence of complications in the study group was 5.13%lower than that in the control group(20.45%),the difference was statistically significant(P<0.05).Conclusion Compared with ordinary locking plate internal fixation,multiaxial locking plate has a more significant therapeutic effect in patients with tibial plateau fractures,improving the range of motion of limbs,shortening the time of operation,hospitalization and fracture healing,and reducing complications.
