摘　　要：目的观察探讨瘢痕子宫产妇的再次妊娠时机和分娩方式的危险因素。方法回顾性分析首都医科大学昌平教学医院2021年7月至2022年12月接收的116例瘢痕子宫产妇的临床资料,分析瘢痕子宫再次妊娠的分娩时机,同时分析影响分娩方式的影响因素。结果116例瘢痕子宫产妇行剖宫产的主要原因是存在剖宫产指征(50.86%),其次是瘢痕厚度<3 mm(39.66%)、孕妇及家属要求(9.48%)。单因素分析发现,距离上次剖宫产时间间隔、BMI、年龄、新生儿体质量是影响瘢痕子宫再次妊娠分娩时机和分娩方式选择的相关因素;多因素Logistic回归分析显示,年龄>35岁、与上次剖宫产间隔时间<2年、BMI>30 kg/m^(2)、新生儿体质量>2.5 kg是影响癫痕子宫再次妊娠分娩时机、分娩方式的危险因素。结论年龄>35岁、BMI>30 kg/m2、与上次剖宫产间隔时间<2年、新生儿体质量>2.5 kg是影响癫痕子宫再次妊娠分娩时机、分娩方式的危险因素。Objective To observe and explore the risk factors of the time of re-pregnancy and delivery mode of pregnant women with scar uterus.Methods The clinical data of 116 cases of pregnant women with scar uterus received from Changping Teaching Hospital of Capital Medical University from July 2021 to December 2022 were retrospectively analyzed,and the delivery time of the second pregnancy of scar uterus was analyzed,and the influencing factors of delivery mode were also analyzed.Results The main reason of cesarean section in 116 cases of pregnant women with scar uterus was the presence of cesarean section indications(50.86%),followed by scar thickness<3 mm(39.66%),and the requirements of pregnant women and their families(9.48%).Univariate analysis showed that the time interval from the last cesarean section,BMI,age and neonatal weight were the related factors affecting the timing of delivery and the choice of delivery mode of scar uterus re-pregnancy;after multivariate logistic regression analysis,it was found that age>35 years old,the interval between the last cesarean section<2 years,BMI>30 kg/m^(2),and neonatal body mass>2.5 kg could all be used as the time to influence the second pregnancy and delivery of epileptic uterus Risk factors of delivery mode.Conclusion Age>35 years old,BMI>30 kg/m2,interval between the last cesarean section<2 years,and neonatal body mass>2.5 kg are the risk factors that affect the delivery time and delivery mode of the second pregnancy of epileptic ute rus.
