摘　　要：目的观察关节镜下内固定治疗复杂胫骨平台骨折的疗效。方法选择78例复杂胫骨平台骨折患者为研究对象,按随机数表法分为观察组(n=39)和对照组(n=39),对照组行传统切开复位内固定术(ORIF),观察组行膝关节镜下内固定术,比较两组患者手术指标切口长度、出血量、术后引流量、手术时间和下床时间,比较术前和术后3个月时两组患者膝关节功能[美国特种医院膝关节评分(HSS)]、骨折愈合效果和并发症发生率。结果观察组切口长度、出血量、术后引流量、手术时间和下床时间均较对照组显著更短差异有统计学意义(P<0.05)。术后3个月时,两组患者HSS评分均较术前明显升高,且同一时间,观察组较对照组显著更高差异有统计学意义(P<0.05)。观察组并发症发生率较对照组明显更低差异有统计学意义(P<0.05)。结论关节镜下内固定术具有手术创口小、并发症少等优势,可以促进复杂胫骨骨折患者膝关节功能恢复,有助于预后。Objective To observe the efficacy of arthroscopic internal fixation in the treatment of complex tibial plateau fractures.Methods A total of 78 patients with complex tibial plateau fractures who were treatedwere selected as the research objects,and divided into the observation group(n=39)and the control group(n=39)according to random number table method.The control group received traditional open reduction and internal fixation(ORIF),and the observation group received knee arthroscopic internal fixation.The differences in surgical indicators(incision length,blood loss,postoperative drainage,operation time and ambulation time)between the two groups were compared.The differences in knee function(HSS),fracture healing effect,and complication rates were compared between the two groups before surgery and 3 months after surgery.Results The incision length,blood loss,postoperative drainage volume,operation time and ambulation time in the observation group were significantly shorter than those in the control group(P<0.05).At 3 months after operation,the HSS score of the two groups was significantly higher than that before operation,and at the same time,the HSS score of the observation group was significantly higher than that of the control group(P<0.05).The incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusions Arthroscopic internal fixation has the advantages of small surgical incision and fewer complications,which can promote the recovery of knee joint function in patients with complex tibial fractures and contribute to prognosis.
