摘　　要：目的探讨Madelung病的影像表现及其诊断价值,提高对该病的认识水平。方法选择22例Madelung病患者,其中男性16例,女性6例;年龄47~77岁,平均年龄58岁。行CT或/和MRI检查,分析其影像学表现,包括脂肪分布情况和Madelung病分型。22例患者中3例行肿块切除,进行病理学检查。结果3例病理诊断结果,光学显微镜下均见肿块为成熟的脂肪组织,其间有少量血管及纤维组织。22例患者病灶发生部位分别为头颈和(或)项背部区域、胸部、臀部及大腿皮下和(或)肌肉间隙、纵隔区域。累及皮下及肌肉间隙,边界欠清,沿筋膜间隙向深部生长,不形成软组织肿块,且无包膜。Madelung病分型显示,Ⅰ型20例,Ⅱ型1例,Ⅲ型1例。结论Madelung病具有特征性的CT、MRI表现,CT、MRI表现在定性诊断方面具有重要价值,对手术治疗方案提供重要参考依据。Objective To investigate the imaging characteristic and diagnostic value of Madelung's disease,and improve the understanding of the disease.Methods A total of 22 patients with Madelung's disease were enrolled,which included 16 males and 6 females,aged 47-77 years old with mean age of 58 years old.The CT or/and MRI were performed to analyze the imaging features of fat distribution and Madelung's disease classification.Among them,3 cases underwent mass resection and pathological examination.Results The pathological diagnosis of 3 cases showed that the mass was mature adipose tissue with small amount of blood vessels and fibrous tissue,under examination of optical microscope.The lesions of 22 patients were located in subcutaneous and/or muscular space of head,neck and/or back,chest,hip and thigh,and mediastinal.The lesions involved subcutaneous and muscular space,the boundary was unclear,and grown deep along the fascial space without soft tissue mass and capsule.According to Madelung's disease classification,there were 20 cases of type I,1 case of type II and 1 case of type II.Conclusion It is demonstrated that Madelung's disease showed characteristic CT and MRI findings.CT and MRI findings play important roles in qualitative diagnosis of Madelung's disease,and provide important reference for surgical treatment.
