摘　　要：[目的]探讨刘春华教授从“脾主运化”治疗2型糖尿病前期的经验。[方法]通过跟师随诊,收集整理典型案例,总结刘教授对2型糖尿病前期的临证心得、遣方用药特色,并列举验案加以分析。[结果]刘教授认为,糖尿病前期应归属于“脾瘅”范畴而非“消渴”,病位当责之于脾,病机总属本虚标实,以脾虚不运为本,兼夹湿热、痰浊、气滞、血瘀等证为标,虚实夹杂、标本并存。临证施治过程中健脾助运贯穿始终,再随证兼顾祛邪,标本同治。所附验案以虚证为主,辨为脾胃虚弱证,治以健脾助运、益气养阴,配合生活方式调整,获得满意疗效。[结论]刘教授重视临床辨证,从“脾主运化”辨治2型糖尿病前期,并提出“健脾助运”为治疗大法,遣方用药重视脾虚之本,兼顾标实,临证疗效甚佳,值得推广学习。[Objective]To discuss Professor LIU Chunhua‘s experience in the treatment of type-2 pre-diabetes from“spleen governing transport”.[Methods]Through following up the teacher,typical cases were collected and sorted,and Professor LIU’s clinical experience and medication characteristics of type-2 pre-diabetes were summarized,and the test case was listed for analysis.[Results]Professor LIU believes that pre-diabetes should belong to the category of“spleen paralysis”rather than“consumptive thirsty”,the disease position should be blamed on the spleen,the pathogenesis is always based on the deficiency of the spleen,and the evidence of humid heat,phlegm,Qi stagnation,blood stasis,etc.,the deficiency and excess mixing,and the manifestation and root cause coexisting.In the process of clinical treatment,the spleen movement is strengthened throughout,and then taken into account for the dispelling of evil,and the manifestation and the root cause are treated together.The attached test case was mainly deficiency syndrome,which was identified as weakness of the spleen and stomach,and the treatment was to strengthen the spleen and help transport,invigorate Qi and nourish Yin,cooperate with lifestyle adjustments,and achieved satisfactory results.[Conclusion]Professor LIU attaches great importance to syndrome differentiation,identify and treat the type 2 pre-diabetes from the“spleen governing transport”,forward the“strengthen the spleen and help transport”as the treatment method,the prescription of medication attaches importance to the basis of spleen deficiency,taking into account the excess manifestation,and has excellent clinical efficacy,which is worth promoting and learning.
