试析《燥气总论》从燥气论治肿瘤放射性损伤

摘　　要：[目的]初步探讨陈葆善《燥气总论》中的燥气理论思想,以期为临床从燥气论治肿瘤放射性损伤提供思路和方法。[方法]通过熟读细析《燥气总论》,试从燥邪的致病特点、发病传变、治法方药等方面,系统总结陈氏燥邪证治的思想内涵,归纳肿瘤放射性损伤的临床特征,探析应用燥气理论治疗肿瘤放射性损伤的潜在学术思路。[结果]《燥气总论》指出,燥邪在病理上兼具寒、火、湿三邪的致病特点,病机传变遵循“先伤肺胃,先气后血,再伤络脉”的规律,在发病方式上又有直中、伏气之别,重视运用辛散之法以治燥邪,尊《伤寒论》之麻杏石甘汤、大青龙汤为治燥祖方。归纳肿瘤放射性损伤的临床特征发现,放射线可归为一种特殊的燥气病邪,其致病亦有火、寒、湿三邪特点,基本病机亦为燥气致病,先气后血,伤及络脉,故可尝试运用《燥气总论》的学术思想论治放射性损伤。[结论]《燥气总论》对燥邪之理法方药分析论述精辟独到,恶性肿瘤放射性损伤亦具有燥邪致病特点,因此运用《燥气总论》中的燥气理论对肿瘤放射性损伤进行治疗具有一定的临床指导价值,值得进一步深入发掘、探讨。[Objective]To discuss the theory of dryness in CHEN Baoshan‘s General Theory of Dry Qi,hoping to provide ideas and methods for the treatment of tumor radiation injury.[Methods]By studying General Theory of Dry Qi,this paper tried to systematically summarize its thoughts from the aspects of dry Qi's pathogenesis,disease changes,treatment methods,summarize the clinical characteristics of tumor radiation injury,and explore the potential academic ideas of applying dry Qi theory to treat tumor radiation injury.[Results]General Theory of Dry Qi pointed out that the pathogenic characteristics of dry pathogens had the pathogenic characteristics of cold,fire and dampness in the pathology,the disease transmission and change followed the law of“first lung,then stomach,first Qi,then blood and then collateral”.There was also a difference in the way of attack including the straight injury and the subdued Qi.In clinic,it should attach great importance to the use of dispersing effect of pungent taste method to treat dryness,and take Maxing Shigan Decoction and Daqinglong Decoction in Treatise on Febrile Diseases as primary formula.After summarizing the clinical characteristics of tumor radiation injury,it was found that radiation can be classified as a special pathogenic pathogen of dry Qi,which also had the characteristics of fire,cold and dampness,and the basic pathogenesis was also caused by dry Qi,first Qi and blood,injury to collaterals,therefore,the academic thought of General Theory of Dry Qi can be used to treat radiation injury.[Conclusion]General Theory of Dry Qi is incisive and unique in the analysis of the principles,methods and prescriptions of dryness pathogens,and tumor radiation injury also has the characteristics of dry pathogens.The treatment has certain clinical guiding value,and it is worthy of further exploration and discussion.
