摘　　要：目的探讨肱骨近端锁定钢板结合同种异体股骨头结构植骨治疗肱骨近端骨折(PHF)的临床疗效。方法回顾性分析我院收治的80例PHF患者的临床资料。所有患者均采用肱骨近端锁定钢板结合同种异体股骨头结构植骨治疗。术后采用肩关节功能评分标准进行评价,随访评估患者骨折愈合情况。结果患者术后随访12~24个月,平均16个月。患者术后均未出现感染、肱骨头坏死、内固定松动、骨不愈合等并发症。骨折临床愈合时间8~24周,平均15周。按肩关节功能评分标准:优60例,良15例,差5例。80例患者术后疼痛、肩关节活动范围、肌力、日常生活活动能力评分均高于术前(P<0.05)。结论肱骨近端锁定钢板结合同种异体股骨头结构植骨治疗PHF效果显著,且无相关并发症发生。Objective To investigate the clinical effect of proximal humeral locking plate combined with allogeneic femoral head structure bone grafting in the treatment of proximal humeral fracture(PHF).Methods The clinical data of 80 PHF patients admitted to our hospital were analyzed retrospectively.All patients were treated with proximal humeral locking plate combined with allogeneic femoral head structure bone grafting.The shoulder joint function score standard was used for postoperative evaluation,and the fracture healing status of patients were evaluated with follow-up.Results The patients were followed up for 12 to 24 months after surgery,with an average of 16 months.There were no complications such as infection,humeral head necrosis,loose internal fixation or nonunion after operation.The clinical healing time of fracture was 8 to 24 weeks,with an average of 15 weeks.According to the shoulder joint function score standard,60 cases were excellent,15 cases were good and 5 cases were poor.The scores of postoperative pain,shoulder joint range of motion,muscle strength and activity of daily living in 80 patients were higher than those before operation(P<0.05).Conclusion Proximal humeral locking plate combined with allogeneic femoral head structure bone grafting is effective in the treatment of PHF without related complications.
