早期胃癌内镜分型与病理学特征及P504s、p53、Ki-67蛋白表达的相关性

摘　　要：目的探讨早期胃癌(GC)患者内镜分型与GC分化程度、浸润深度、P504s、肿瘤蛋白p53、Ki-67的相关性。方法回顾性分析2020年1月至2022年6月本院收治的100例早期GC患者的临床资料,分析早期GC内镜分型与病变部位、分化程度、浸润深度及P504s、p53、Ki-67蛋白表达的相关性。结果早期GC患者中胃窦部位病变率最高(55%),胃角病变率次之(20%),胃体、胃窦、贲门、胃角等病变部位多以Ⅲ型、Ⅱc型为主。早期GC患者中分化者的占比68%高于未分化者的32%(P<0.05),且未分化者以Ⅱb型为主,分化者以Ⅲ型为主。早期GC患者中浸润黏膜层者占56%,浸润黏膜下层者占44%,且浸润黏膜层者以Ⅱc型为主,浸润黏膜下层者以Ⅲ型为主。早期GC患者中P504s、p53、Ki-67阳性占比分别为80%、78%、95%,且均以Ⅲ型为主。经Spearman相关性分析显示,内镜分型与P504s、p53、Ki-67蛋白表达均呈正相关(r=0.310、0.308、0.349;P=0.034、0.042、0.028)。结论早期GC患者内镜分型与分化程度、浸润深度及P504s、p53、Ki-67蛋白表达水平有关,内镜分型越高则分化程度及浸润深度越严重,P504s、p53、Ki-67蛋白表达水平越高,确定以上指标有助于临床诊治早期GC。Objective To investigate the correlation between endoscopic classification of patients with early gastric cancer(GC)and the degree of GC differentiation,depth of invasion,P504s,tumor proteins p53 and Ki-67.Methods The clinical data of 100 early GC patients admitted to our hospital from January 2020 to June 2022 were retrospectively analyzed,and the correlation between endoscopic classification of early GC and lesion location,degree of differentiation,depth of invation,expression of P504s,p53 and Ki-67 proteins were analyzed.Results In the early GC patients,the lesion rate in gastric antrum was the highest(55%),followed by the gastric angle(20%),and the lesions of gastric body,gastric antrum,cardia and gastric angle were mostly typeⅢand typeⅡc.The proportion of differentiated patients in early GC was 68%,higher than 32%of undifferentiated patients(P<0.05),and the undifferentiated patients were mainly typeⅡb,and the differentiated patients were mainly typeⅢ.Among the early GC patients,56%had mucosal infiltration,and 44%had submucosa infiltration,the typeⅡc was the main type of mucosal infiltration,and the typeⅢwas the main type of submucosa infiltration.The positive proportions of P504s,p53 and Ki-67 in early GC patients were 80%,78%and 95%respectively,and all of them were mainly typeⅢ.Spearman correlation analysis showed that endoscopic classification was positively correlated with the expression of P504s,p53 and Ki-67 proteins(r=0.310,0.308,0.349;P=0.034,0.042,0.028).Conclusion Endoscopic classification of early GC patients is related to the degree of differentiation,depth of invasion,and expression levels of P504s,p53,and Ki-67 proteins.The higher the endoscopic classification,the more severe the degree of differentiation and depth of invasion,and the higher the expression levels of P504s,p53,and Ki-67 proteins.Determination of above indicators is helpful for clinical diagnosis and treatment of early GC.
