摘　　要：目的探讨经膝关节前方正中入路内固定治疗股骨远端C2C3型骨折的临床效果。方法选择2019年4月至2021年4月遵义医科大学附属医院收治的11例股骨远端粉碎性骨折患者为研究对象,其中6例患者经膝关节前方正中入路双钢板内固定治疗为观察组,5例患者经内外侧双切口入路双钢板内固定治疗为对照组。对比分析2组股骨髁关节面的暴露程度、手术时间、术中出血量、术后引流量及术后膝关节功能;评价经膝关节前方正中入路内固定治疗股骨远端C2C3型骨折的效果。结果经膝关节前方正中入路股骨髁关节面均可满意暴露。观察组术中出血量及手术时间均少/短于对照组,差异均有统计学意义(P<0.05),术后引流量及膝关节功能评分与对照组相比无统计学差异(P>0.05)。结论经膝关节前方正中入路内固定治疗股骨远端C2C3型骨折效果显著,具有暴露充分、手术时间短、出血量少等优点。Objective To explore the clinical effect of internal fixation via knee anterior median approach in the treatment of type C2C3 fractures of distal femur.Methods A total of 11 patients with comminuted fracture of distal femur in Affiliated Hospital of Zunyi Medical University from April 2019 to April 2021 were selected as the study objects,of which 6 patients were treated with double plate internal fixation via knee anterior median approach as the observation group,and 5 patients were treated with double plate internal fixation via internal and external incisions approach as the control group.The exposure degree of articular surface of femoral condyle,operation time,intraoperative blood loss,postoperative drainage volume and postoperative knee joint function were compared between the two groups.The effect of internal fixation via knee anterior median approach for type C2C3 fractures of distal femur was evaluated.Results The articular surface of femoral condyle can be exposed satisfactorily through the knee anterior median approach.The operation time and intraoperative blood loss of the observation group were less/shorter than those of the control group(P<0.05),and there was no significant difference in postoperative drainage volume and knee function scores between the two groups(P>0.05).Conclusion The treatment of type C2C3 fractures of distal femur by internal fixation via knee anterior median approach has good effect with the advantages of full exposure,shorter operation time and less bleeding.
