探究后路经皮内镜颈椎间盘切除术在神经根型颈椎病治疗中的效果

摘　　要：目的:探究在神经根型颈椎病(Cervical spondylotic radiculopathy,CSR)的治疗工作中应用后路经皮内镜颈椎间盘切除术(Posterior percutaneous endoscopic cervical Discectomy,PPECD)的有效率及价值。方法:回顾性方式进行本次分析,研究时段介于2021年3月-2022年5月,将我院23例CSR患者予以选取并给予PPECD。观察并评价患者治疗前后神经系统功能、上肢及颈部疼痛情况以及颈椎曲度改善情况。结果:(1)本研究纳入23例CSR患者均给予PPECD治疗,患者手术耗时、术后住院时间较短,术中出血量较少,且术后恢复良好,并无硬膜囊破裂、神经根损伤等并发症发生。(2)与术前进行比较,术后患者上肢VAS、JOA评分出现较大幅度的改变,且数据对比存在明显差异(P<0.05)。(3)患者术后颈椎曲度、手术节段ROM和术前数据进行比较,数值无显著差异,统计学不成立(P>0.05)。结论:在CSR患者的治疗工作中使用PPECD效果显著,术中各项操作并不会影响患者术后颈椎曲度,可提高临床疗效的同时改善患者预后。Objective:To explore the role of Cervical spondylotic radiculopathy in Cervical spondylotic radiculopathy,The effectiveness and value of Posterior percutaneous endoscopic cervical Discectomy(PPECD)in the treatment of (CSR).Methods:This analysis was conducted retrospectively,and the study period was from March 2021 to May 2022.23 patients with CSR in our hospital were selected and given PPECD.Neurological function,upper limb and neck pain,and improvement of cervical curvature were observed and evaluated before and after treatment.Results:(1)All the 23 CSR patients included in this study received PPECD treatment,and the patients had a long operation time,a short postoperative hospital stay,less intraoperative blood loss,and a good postoperative recovery,without dural sac rupture,nerve root injury and other complications.(2)Compared with the preoperative results,the postoperative upper extremity VAS and JOA scores of patients showed significant changes,and the data were significantly different(P<0.05).(3)There was no significant difference between postoperative cervical curvature,surgical segment ROM and preoperative data(P>0.05).Conclusion:The use of PPECD in the treatment of patients with CSR has a significant effect,and the intraoperative operations do not affect the postoperative cervical curvature of patients,which can improve the clinical efficacy and the prognosis of patients.
