摘　　要：目的研讨颈动脉内膜切除术术前经颅彩色多普勒超声(TCCD)联合颈动脉超声进行风险评估的价值。方法回顾性分析新乡医学院2018年1月至2022年10月收治的86例行颈动脉内膜切除术的颈动脉狭窄患者,在手术前全部予以TCCD联合颈动脉超声进行风险评估,探讨其临床应用价值。结果86例行颈动脉内膜切除术的颈动脉狭窄患者术后1个月内均无死亡,共有13例患者出现了并发症(15.12%),包含6例过度灌注综合征、5例新发脑梗死和2例脑出血。经Logistic回归分析发现年龄、颈总动脉分叉位置、是否合并对侧颈动脉重度狭窄或闭塞、大脑中动脉内收缩期峰值流速、侧支循环均为颈动脉狭窄患者颈动脉内膜切除术后是否出现并发症的影响因素。结论颈动脉内膜切除术前应用TCCD、颈动脉超声对血管病变的血流动力学和局部结构进行全面评估,能预测术后可能发生的并发症,进而更好地完善术前准备。Objective To study the value of transcranial color doppler ultrasound(TCCD)combined with carotid artery ultrasound in risk assessment before carotid endarterectomy.Methods A retrospective analysis was performed on 86 patients with carotid artery stenosis who underwent carotid endarterectomy in Xinxiang Medical College from January 2018 to October 2022.All patients were given TCCD combined with carotid artery ultrasound for risk assessment before surgery,and its clinical application value was discussed.Results There was no death within 1 month after carotid endarterectomy in 86 patients with carotid stenosis.A total of 13 patients developed complications(15.12%),including 6 cases of hyperperfusion syndrome,5 cases of new cerebral infarction,and 2 cases of cerebral hemorrhage.Logistic regression analysis showed that age,bifurcation location of the common carotid artery,severe stenosis or occlusion of the contralateral carotid artery,peak systolic flow velocity in the middle cerebral artery and collateral circulation were the influencing factors of complications after carotid endarterectomy in patients with carotid artery stenosis.Conclusions The application of TCCD and carotid ultrasound before carotid endarterectomy can comprehensively evaluate the hemodynamics and local structure of vascular lesions,predict the possible postoperative complications,and better improve the preoperative preparation.
