八段锦对强直性脊柱炎患者异位骨化及白细胞介素-23水平的影响

摘　　要：目的:观察八段锦对强直性脊柱炎患者临床疗效及异位骨化、白细胞介素-23(IL-23的影响。方法:将40例强直性脊柱患者随机分为观察组和对照组,每组20例。对照组予以塞来昔布胶囊口服治疗,观察组在对照组基础上规律练习八段锦,锻炼6个月。观察2组治疗前后Bath强直性脊柱炎疾病活动度指数(BASDAI)、强直性脊柱炎新疾病活动度评分(ASDAS)、疼痛视觉模拟评分法(VAS)评分、指地距、Schober试验、炎症指标[C反应蛋白(CRP)、红细胞沉降率(ESR)]、焦虑抑郁量表(SAS、SDS评分)、IL-23、Dickkopf-1(DKK-1)、骨保护素(OPG)。结果:治疗后,与治疗前比较,2组BASDAI、ASDAS、VAS评分、指地距、CRP、ESR、IL-23均显著降低(P<0.05或P<0.01),Schober试验、DKK-1显著升高(P<0.05或P<0.01);观察组BASDAI、ASDAS、VAS评分、指地距、SAS、SDS均显著低于对照组(P<0.05或P<0.01),DKK-1、OPG显著高于对照组(P<0.05或P<0.01)。结论:八段锦能降低强直性脊柱炎患者的疾病活动度,改善炎症反应,抑制异位骨化,提高关节活动功能,缓解焦虑抑郁情绪,便于临床推广。Objective:To observe the influence of Baduanjin(a kind of martial arts)on the clinical efficacy of ankylosing spondylitis,heterotopic ossification and interleukin-23(IL-23).Methods:Forty patients with ankylosing spondylitis were randomly divided into a observation group and a control group,with 20 cases in each group.The control group was given celecoxib capsule orally,and the observation group regularly practiced Baduanjin for 6 months based on the treatment as that for the control group.The BASDAI,ASDAS,VAS,fingerground distance,Schöber test,inflammatory indicators(CRP,ESR),anxiety and depression scale(SAS,SDS),IL-23,DKK-1 and OPG were observed before and after treatment in the two groups.Results:After treatment,BASDAI,ASDAS,VAS,finger-ground distance,CRP,ESR,IL-23 in both groups were significantly reduced(P<0.05 or P<0.01),while Schöber test and DKK-1 were significantly increased(P<0.05 or P<0.01)compared with those before treatment.The BASDAI,ASDAS,VAS,finger-ground distance,SAS,and SDS in the observation group were signifi-cantly lower than those in the control group(P<0.05 or P<0.01),while DKK-1 and OPG were significantly higher than those in the control group(P<0.05 or P<0.01).Conclusion:Baduanjin can reduce the disease activity of patients with ankylosing spondylitis,improve inflammatory reaction,inhibit heterotopic ossification,improve joint activity,alleviate anxiety and depression,and facilitate clinical promotion.
