摘  要:目的调查苯接触对作业工人血常规指标的影响。方法采用横断面调查,选择2017-2021年武汉市6398名苯接触工人为观察组,以工作中不接触苯及其他有毒有害物质的1497名行政办公人员为对照组。对两组人员白细胞计数、中性粒细胞计数、血小板计数、红细胞计数、血红蛋白水平进行比较分析,分析影响血常规指标下降的危险因素。结果观察组工人白细胞计数比血小板计数均值低于对照组,差异有统计学意义(t值=2.599、5.933,P<0.05)。接触苯的女性白细胞计数、中性粒细胞计数、红细胞计数、血红蛋白计数均值均低于男性,差异有统计学意义(t=-5.563、-3.002、-54.673、-64.871,P<0.05)。多因素logistic回归分析结果显示,苯是白细胞计数偏低的主要影响因素(β=2.321,OR=10.18),其次为性别、年龄。白细胞计数偏低的风险不随接害工龄的增加而增加(β=-0.033,OR=0.968)。结论低浓度苯接触存在对工人血常规指标的影响,对白细胞降低影响较大,其次对血小板计数和中性粒细胞计数的降低也有不同程度的影响,应重视女工的保护。Objective To investigate the effect of benzene exposure on blood routine indexes of workers during 2017-2021.Methods A cross-sectional survey was conducted.A total of 6398 workers exposed to benzene in Wuhan from 2017 to 2021 were selected as the observation group,and 1497 office administrative workers who were not exposed to benzene and other toxic and harmful substances were selected as the control group.The white blood cell count,neutrophil count,platelet count,red blood cell count and hemoglobin level of the two groups of workers were compared and analyzed,and the risk factors affecting the decline of blood routine indexes were analyzed.Results The white blood cell count and platelet count of the observation group were lower than those of the control group.The difference was statistically significant(t-2.599,5.933,P<0.05).White blood cell count,neutrophil count,mean red blood cell count and hemoglobin count of female workers exposed to benzene were lower than those of men.The differences were statistically significant(t=-5.563,-3.002,-54.673,-64.871,P<0.05).The results of multivariate logistic regression analysis showed that benzene exposure was the main influencing factor of low white blood cell count(β=2.321,OR=10.18),followed by gender and age.The risk of low white blood cell count did not increase with the increase of exposure time(β=-0.033,OR=0.968).Conclusion Low concentration of benzene exposure has a great influence on the blood routine indexes of workers,and has a great influence on the reduction of leukopenia,and then has a different degree of influence on the reduction of platelet count and neutrophil count,especially the protection of female workers should be paid attention to.
