健脾温肾汤联合西药治疗亚临床甲状腺功能减退症临床研究

摘　　要：目的:观察健脾温肾汤联合西药治疗亚临床甲状腺功能减退症的效果及对甲状腺功能、脂代谢及心功能的影响。方法:选择110例亚临床甲状腺功能减退症患者为研究对象,按随机数字表法分为对照组与研究组各55例。对照组给予左甲状腺素钠片治疗,研究组在对照组基础上给予健脾温肾汤治疗。比较2组治疗前后血清游离四碘甲状腺原氨酸(FT4)、游离三碘甲状腺原氨酸(FT3)、促甲状腺激素(TSH)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、三酰甘油(TG)水平及左心室射血分数(LVEF)、二尖瓣口A峰及E峰血流速度,并比较2组临床疗效。结果:研究组治疗总有效率96.36%,高于对照组81.82%(P<0.05)。治疗前,2组血清FT3、FT4、TSH水平比较,差异无统计学意义(P>0.05);治疗后,2组血清FT3、FT4水平升高(P<0.05),且研究组高于对照组(P<0.05),2组血清TSH水平降低(P<0.05),且研究组低于对照组(P<0.05)。治疗前,2组血清TC、TG、LDL-C、HDL-C水平比较,差异无统计学意义(P>0.05);治疗后,2组血清TC、TG、LDL-C水平降低(P<0.05),且研究组低于对照组(P<0.05),2组血清HDL-C水平升高(P<0.05),且研究组高于对照组(P<0.05)。治疗前,2组LVEF、A峰血流速度、E峰血流速度比较,差异无统计学意义(P>0.05);治疗后,2组A峰血流速度降低(P<0.05),且研究组低于对照组(P<0.05),2组LVEF、E峰血流速度升高(P<0.05),且研究组高于对照组(P<0.05)。结论:健脾温肾汤联合西药治疗亚临床甲状腺功能减退症,可改善患者甲状腺功能,调节患者血脂代谢水平,改善患者心功能,提高临床疗效。Objective:To observe the clinical effect of Jianpi Wenshen Decoction combined with western medicine for subclinical hypothyroidism and its effect on thyroid function,lipid metabolism,and heart function.Methods:A total of 110 cases of patients with subclinical hypothyroidism were selected as research objects and divided into the control group and the study group according to the random number table method,with 50 cases in each group.The control group was treated with Levothyroxine Sodium Tablets,and the study group was additionally treated with Jianpi Wenshen Decoction based on the treatment of the control group.The levels of free tetraiodothyronine(FT4),free triiodothyronine(FT3),thyroid stimulating hormone(TSH),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),total cholesterol(TC),and triacylglycerol(TG)in serum,left ventricular ejection fraction(LVEF),and blood flow velocity at peak A and peak E of mitral orifice were compared before and after treatment between the two groups;the clinical effects were compared between the two groups.Results:The total effective rate of treatment was 96.36%in the study group,higher than that of 81.82%in the control group(P<0.05).Before treatment,there was no significant difference being found in the comparison of the levels of FT3,FT4,and TSH in serum between the two groups(P>0.05).After treatment,the levels of FT3 and FT4 in serum in the two groups were increased(P<0.05),and the above levels in the study group were higher than those in the control group(P<0.05);the TSH levels in serum in the two groups were decreased(P<0.05),and the TSH level in the study group was lower than that in the control group(P<0.05).Before treatment,there were no significant difference being found in the comparison of the levels of TC,TG,LDL-C,and HDL-C in serum between the two groups(P>0.05).After treatment,the levels of TC,TG,and LDL-C in serum in two groups were decreased(P<0.05),and the above levels in the study group were lower than those in the control group(
