MEBT/MEBO与PRP治疗糖尿病慢性溃疡的疗效对比分析

摘　　要：目的 对比分析创疡再生医疗技术(MEBT/MEBO)与富血小板血浆(PRP)治疗糖尿病慢性溃疡的临床疗效。方法 选取2020年7月至2021年7月郑州大学第一附属医院收治的100例糖尿病溃疡患者作为研究对象,并按照不同治疗方法将其分为MEBT/MEBO组(50例)与PRP组(50例),MEBT/MEBO组患者局部创面予以MEBT/MEBO治疗,PRP组患者局部创面予以PRP治疗,对比观察两组患者创面渗液pH值、疼痛程度、愈合情况以及临床疗效。结果 治疗第5、10天,MEBT/MEBO组患者创面渗液pH值、五指疼痛评分法(FFM)评分、Bates-Jensen伤口评估与效果评价量表(BWAT)评分均明显低于PRP组(治疗第5天:t=4.030、3.269、5.512,P<0.001、P=0.002、P<0.001;治疗第10天:t=6.790、4.889、5.540,P均<0.001)。治疗4周后,MEBT/MEBO组患者创面治愈14例、显效16例、有效17例、无效3例,明显优于PRP组患者的创面治愈8例、显效14例、有效20例、无效8例(Z=-1.965,P=0.049)。结论 与PRP相比,MEBT/MEBO更能有效降低糖尿病慢性溃疡创面pH值,减轻创面疼痛,疗效更好。Objective To compared the clinical efficacy of the regenerative medical technology for wounds and ulcers(MEBT/MEBO) and platelet-rich plasma(PRP) in the treatment of diabetic chronic ulcer.Methods 100 patient with diabetic ulcer admitted into The First Affiliated Hospital of Zhengzhou University between July 2020 and July 2021 were enrolled as research subjects,and divided into MEBT/MEBO group(n=50) and PRP group(n=50) according to different treatments they received.The local wounds of patients in the MEBT/MEBO group were treated with MEBT/MEBO,while the local wounds of patients were treated with PRP in the PRP group.The pH value of wound exudate,pain degree,wound healing condition and clinical efficacy were compared between the two groups.Results Respectively on day 5 and10 of treatment,the pH values of wound exudate,five-finger measure(FFM) scores,scores of Bates-Jensen wound assessment tool(BWAT) of patients in the MEBT/MEBO group were all markedly lower than that in the PRP group(day 5 of treatment:t=4.030,3.269 and 5.512,P 0.001,P=0.002,P 0.001;day 10 of treatment:t=6.790,4.889 and5.540,all P 0.001).After 4 weeks of treatment,the wounds of 14 cases were cured,16 cases showed markedly effective,17 cases were effective and 3 cases were ineffective in the MEBT/MEBO group,being obviously better than that in the PRP group(Z=-1.965,P=0.049)-8 cases cured,14 cases markedly effective,20 cases effective and 8 cases ineffective.Conclusion Compared with PRP,the MEBT/MEBO is much better in decreasing pH value of wound exudate of diabetic chronic ulcer and alleviating wound pain,realizing better clinical efficacy.
