摘　　要：目的探讨急性胰腺炎患者住院期间发生下肢深静脉血栓(DVT)的影响因素。方法收集2021年3月至2022年3月于首都医科大学附属北京友谊医院住院接受治疗的127例急性胰腺炎患者的临床资料,按照住院期间是否发生下肢DVT将其分为血栓组(n=26)和对照组(n=101)。收集所有患者入院时的临床特征、实验室检测结果、合并症等,采用Logistic回归模型分析急性胰腺炎患者发生下肢DVT的影响因素。结果血栓组患者的年龄、体重指数、重症胰腺炎发生率、D-二聚体水平均高于对照组患者,差异均有统计学意义(P﹤0.05)。多因素分析结果显示,年龄≥60岁、体重指数﹥24 kg/m^(2)、重症胰腺炎、D-二聚体﹥0.5 mg/L均是急性胰腺炎患者发生下肢DVT的危险因素。结论年龄≥60岁、体重指数﹥24 kg/m^(2)、重症胰腺炎、D-二聚体水平﹥0.5mg/L均是急性胰腺炎患者发生下肢DVT的危险因素,临床中应密切关注患者的上述因素。Objective To analyze the risk factors of lower extremity deep vein thrombosis(DVT)in patients with acute pancreatitis during hospitalization.Method Clinical data of 127 patients with acute pancreatitis who were hospitalized in Beijing Friendship Hospital,Capital Medical University from March 2021 to March 2022 were collected.The patients were divided into thrombosis group(n=26)and control group(n=101)according to occurrence of lower extremity DVT during hospitalization.The clinical characteristics on admission,laboratory test results and complications were collected,and the risk factors of lower extremity DVT in patients with acute pancreatitis were analyzed by Logistic regression model.Result The age,body mass index,the rate of severe pancreatitis,and the levels of D-dimer in the thrombosis group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).Multivariate showed that,age≥60 years old,body mass index>24 kg/m^(2),severe pancreatitis,and D-dimer>0.5 mg/L were risk factors of lower extremity DVT in patients with acute pancreatitis.Conclusion Age≥60 years old,body mass index>24 kg/m^(2),severe pancreatitis,D-dimer>0.5 mg/L are risk factors for lower extremity DVT in patients with acute pancreatitis,and should be noticed in clinic.
