术中综合干预对高位结扎联合激光及硬化剂治疗大隐静脉曲张的影响

摘　　要：目的探讨术中综合干预对高位结扎联合激光及硬化剂治疗大隐静脉曲张的影响。方法收集2021年1月至2022年6月于皖西卫生职业学院附属医院(六安市第二人民医院)接受高位结扎联合激光及硬化剂治疗的100例大隐静脉曲张患者的临床资料,按照治疗方法的不同将其分为对照组(n=49,就诊时间为2021年1月至2021年10月,进行常规术中干预)和综合干预组(n=51,就诊时间为2021年11月至2022年6月,在对照组基础上接受综合干预)。比较两组患者的手术时间、术中出血量、术后并发症发生情况。术后随访3个月,记录两组患者二次手术率、临床表现-病因学-解剖学-病理生理学(CEAP)分级中C5级溃疡面积。结果综合干预组患者手术时间短于对照组患者,术中出血量少于对照组患者,术后并发症发生率低于对照组患者,差异均有统计学意义(P﹤0.05)。术后3个月,综合干预组患者二次手术率为3.9%(2/51),低于对照组患者的18.4%(9/49),差异有统计学意义(P﹤0.05)。干预后,两组患者C5级溃疡面积均小于本组干预前,且综合干预组患者C5级溃疡面积小于对照组患者,差异均有统计学意义(P﹤0.05)。结论高位结扎联合激光及硬化剂治疗大隐静脉曲张的术中综合干预可缩短手术时间,减少术中出血量,降低术后并发症发生率,缩小溃疡面积,降低术后3个月的二次手术率。Objective To investigate the effect of the intraoperative comprehensive intervention on high ligation combined with laser and sclerosing agents in the treatment of great saphenous varicose vein.Method The clinical data of 100 patients with saphenous varicose veins who received high ligation combined with laser and sclerotherapy in the Affiliated Hospital of West Anhui Health Vocational College(Lu'an Second People's Hospital)from January 2021 to June 2022 were collected.According to different treatment methods,they were divided into control group(n=49,visiting time from January 2021 to October 2021,routine intraoperative intervention)and comprehensive intervention group(n=51,visiting time from November 2021 to June 2022,comprehensive intervention based on the control group).The operation time,intraoperative blood loss,and postoperative complications were compared between the two groups.After operation three months of follow-up,the reoperation rate and the area of C5 grade ulcers in the clinical clinical-etiology-anatomy-pathophysiology(CEAP)classification of the two groups were recorded.Result The operation time of the patients in the comprehensive intervention group was shorter than that of the control group,the intraoperative blood loss was less than that of the control group,the incidence of postoperative complications was lower than that of the control group,the differences were statistically significant(P<0.05).After operation three months,the reoperation rate of patients in the comprehensive intervention group was 3.9%(2/51),lower than 18.4%(9/49)of patients in the control group,the difference was statistically significant(P<0.05).After the intervention,the area of C5 grade ulcers in the two groups was smaller than that before the intervention,and the area of C5 grade ulcers in the comprehensive intervention group was smaller than that in the control group,the differences were statistically significant(P<0.05).Conclusion The intraoperative comprehensive intervention of high ligation combined with laser and s
