摘　　要：目的探讨强化降糖治疗对糖尿病足(DF)患者足部症状、溃疡面愈合及心理状态的影响。方法收集2017年4月至2022年5月于秦皇岛市第一医院就诊的90例DF患者的临床资料,根据降糖治疗控制目标不同将其分为常规组(n=45)和强化组(n=45)。比较两组患者治疗前后的足部症状功能情况、创面情况及心理状态。结果治疗后,强化组患者足背动脉搏动、下肢麻木、足部疼痛、皮温温度、皮肤色泽及间歇性跛行缓解时间评分均低于常规组患者,差异均有统计学意义(P﹤0.05)。治疗后,强化组患者的溃疡面积、溃疡深度、肉芽生长、渗液评分均低于常规组患者,差异均有统计学意义(P﹤0.05)。治疗后,强化组患者焦虑自评量表(SAS)、抑郁自评量表(SDS)评分均低于常规组患者,差异均有统计学意义(P﹤0.05)。结论强化降糖治疗可提高DF患者的治疗效果,改善患者足部临床症状,促进溃疡创面愈合,缓解患者的不良情绪。Objective To investigate the effect of intensive hypoglycemic therapy on foot symptoms,ulcer surface healing and psychological state in diabetic foot(DF)patients.Method The clinical data of 90 DF patients in the First Hospital of Qinhuangdao from April 2017 to May 2022 were collected.They were divided into conventional group(n=45)and intensive group(n=45)according to the goals of hypoglycemic therapy.The functional condition of foot symptoms and wounds,and psychological state of the two groups before and after the treatment were compared.Result After treatment,the scores of dorsal foot artery pulse,lower limb numbness,foot pain,skin temperature temperature,skin color,and intermittent claudication relief time in the intensive group were lower than those in the conventional group,the differences were statistically significant(P<0.05).After treatment,the ulcer area,ulcer depth,granulation growth,and exudate scores of the intensive group were lower than those in the conventional group,the differences were statistically significant(P<0.05).After treatment,the scores of self-rating anxiety scale(SAS)and self-rating depression scale(SDS)in the intensive group were lower than those in the conventional group,the differences were statistically significant(P<0.05).Conclusion Intensive hypoglycemic therapy could improve the treatment effect of DF patients,improve the clinical symptoms of feet,promote ulcer wound healing,and relieve their negative emotion.
