儿童幽门螺杆菌相关性胃炎与胃黏膜闭锁蛋白关系的研究

摘　　要：目的探讨幽门螺杆菌(Hp)相关性胃炎患儿感染的Hp类型、胃黏膜闭锁蛋白阳性表达率及胃黏膜炎症程度间的关系。方法选取2018年6月—2021年12月于我院接受胃镜检查并且确诊为Hp相关性胃炎的90例患儿作为研究对象。采用免疫印迹法检测患儿血清Hp抗体表达情况,对Hp进行血清学菌株分型,并观察患儿胃黏膜病理变化;采用免疫组织化学定量法检测胃黏膜闭锁蛋白阳性表达率,分析Hp菌株类型、胃黏膜闭锁蛋白阳性表达率及胃黏膜炎症程度间的关系。结果Ⅰ型菌株感染患儿胃黏膜闭锁蛋白阳性表达率显著低于Ⅱ型及中间型(P<0.05),Ⅱ型与中间型比较无显著差异(P>0.05)。不同Hp菌株感染患儿胃黏膜炎症程度有显著差异(H=18.013,P<0.01),Ⅱ型轻于Ⅰ型及中间型(P<0.05),Ⅰ型与中间型差异无统计学意义(P>0.05)。胃黏膜轻、中、重度炎症患儿的闭锁蛋白阳性表达性率分别为80.0%、35.1%和11.6%,3种炎症程度患儿闭锁蛋白阳性表达率均有显著差异(χ^(2)=19.655,P<0.05)。结论Hp相关性胃炎患儿的胃黏膜闭锁蛋白阳性表达率与Hp菌株类型及胃黏膜炎症程度有关,Ⅰ型菌株感染和胃黏膜重度炎症时闭锁蛋白阳性表达率最低;不同类型菌株感染所致胃黏膜炎症程度不同,Ⅱ型菌株感染所致胃黏膜炎症程度最轻。Hp抗体分型检测或可预测胃黏膜紧密连接和炎症程度,胃黏膜闭锁蛋白有望成为治疗Hp相关性胃炎的重要靶点。Objective To explore the relationship between Helicobacter pylori(Hp)type,positive expression rate of gastric mucosal occludin and degree of gastric mucosal inflammation in children with Hp-associated gastritis.Methods Ninety children who received gastroscopy and were diagnosed with Hp-associated gastritis in our hospital from June 2018 to December 2021 were selected as the research object.The expression of serum Hp antibody in children was measured by Western blot.The strains of Hp were serotyped.The pathological changes of gastric mucosa in children were observed.The positive expression rate of gastric mucosal occludin was determined by quantitative immunohistochemistry method.The relationship between Hp strain type,positive expression rate of gastric mucosal occludin and degree of gastric mucosal inflammation was analyzed.Results The positive expression rate of gastric mucosal occludin in children infected with type Ⅰ strain was significantly lower than that in children infected with type Ⅱ strain or intermediate type strain(P<0.05),but there was no significant difference in the positive expression rate between children infected with type Ⅱ strain and those infected with intermediate type strain(P>0.05).There were significant differences in the degree of gastric mucosal inflammation among children infected with different Hp strains(H=18.013,P<0.01);the degree was significantly less severe in children infected with type Ⅱ strain than in children infected with type Ⅰ strain or intermediate type strain(P<0.05),but there was no significant difference in the degree between children infected with typeⅠstrain and those infected with intermediate type strain(P>0.05).The positive expression rates of occludin in children with mild,moderate,and severe gastric mucosal inflammation were 80.0%,35.1%,and 11.6%,respectively(χ^(2)=19.655,P<0.05).Conclusion The positive expression rate of occludin in the gastric mucosa of children with Hp-associated gastritis is related to the type of Hp strain and the degree of ga
