高压电烧伤致大面积头皮缺损、颅骨外露创面修复1例

摘　　要：高压电导致的严重烧创复合伤伤情重,救治难度大。该文报道了1例万伏高压电流造成的头颅特大面积Ⅲ度烧伤、颅骨外露、局部颅骨碳化合并脑脊液漏的成功救治过程。伤者早期创面经多次有限清创后,予负压封闭引流保护创面,预防感染;择期清理失活颅骨外板,后期采用双侧股前外侧皮瓣结合颅骨钻孔修复创面。患者术后无严重并发症,治疗效果满意。It is difficult to treat severe burn combined injury caused by high voltage.This paper reports the successful treatment of a patient with third degree burn of extra large cranial area,skull exposure,local skull carbonization combined with cerebrospinal fluid leakage caused by ten thousand volts high voltage current.Vacuum sealing drainage was given to protect the wounds from infection after multiple limited debridement in the early stage.The external plate of the inactivated skull was cleaned in the later stage.Finally,the bilateral anterolateral thigh flap combined with multiple burr holes was used to repair the wound.There were no severe postoperative complications,and the therapeutic effect was satisfactory.
