摘　　要：目的:探讨1种用于口腔黏膜卷管的"烟卷芯"实用新型专利成果在尿道下裂修复术中的实践效果。方法:回顾性分析2018年12月至2022年1月中国医学科学院整形外科医院尿道下裂整形科收治的尿道下裂患者临床资料。患者分为2组,均进行分期手术修复尿道下裂:一期手术以口腔黏膜预构尿道,采取口腔黏膜长度为5.0~6.0 cm,宽为1.0~1.2 cm,对照组预构尿道时采用术中缝制的"烟卷芯",试验组采用批量生产的实用新型专利产品"烟卷芯";二期手术行尿道吻接,并应用阴囊皮瓣覆盖创面。比较对照组和试验组尿道下裂一期手术的时长,并随访术后黏膜成活情况、并发症情况、排尿情况及患者对手术效果满意度。正态分布计量资料以±s表示,采用独立样本t检验进行分析。结果:共纳入107例男性患者,对照组53例,试验组54例,年龄1~28岁,平均8.5岁。对照组一期手术时长明显大于试验组[(61.5±11.5)min vs.(43.8±15.2)min],差异有统计学意义(P<0.01)。术后随访3~20个月。一期手术对照组口腔黏膜成活率为98.11%(52/53);试验组口腔黏膜成活率为100%(54/54)。二期术后对照组并发尿瘘4例,并发症发生率为7.55%;试验组并发尿瘘1例,并发症发生率为1.85%。患者术后排尿状况均良好。术后1年满意度调查时,对照组满意率为94.34%(50/53),试验组为100%(54/54)。结论:该实用新型专利"烟卷芯"的一致性好,将其应用于尿道下裂修复手术时,黏膜移植成活率高,术后并发症发生率低,可以有效节约人力资源,减少手术时间和麻醉时间。Objective To evaluate the clinical application of the new"cigarette core"product for oral mucosa roll in hypospadias patients.MethodsFrom December 2018 to January 2022,patients who visited the Plastic Surgery Hospital,Chinese Academy of Medical Sciences and Peking Union Medical College were selected and underwent staged surgical repair.In the first stage,we used the oral mucosa to form the urethra.The length of the oral mucosa was 5.0-6.0 cm and the width was 1.0-1.2 cm.In the control group,we use the handmade gauze roll;in the experimental group,we use the new"cigarette core"product.In the second-stage operation,the urethra was anastomosed.The operation time was recorded,and the mucosal survival,complications,urination and patients’satisfaction with the operative effect were followed up.The measurement data of normal distribution were expressed as Mean±SD and analyzed by t-test.ResultsA total of 107 male patients were included,including 53 in the control group and 54 in the experimental group,aged from 1 to 28 years,with an average age of 8.5 years.The first-stage operation time of control group was significantly longer than that of experimental group[(61.5±11.5)min vs.(43.8±15.2)min],and the difference was statistically significant(P<0.01).The patients were followed up for 3 to 20 months.In the first-stage operation,the survival rate of oral mucosa was 98.11%(52/53)in the control group and 100%(54/54)in the experimental group.There were 4 cases of urinary fistula in the control group after the second-stage operation,and the complication rate was 7.55%.There was 1 case of urinary fistula in the experimental group,and the complication rate was 1.85%.All patients had good urination after operation.In the satisfaction survey one year after surgery,the satisfaction rate of the control group was 94.34%(50/53)and that of the experimental group was 100%(54/54).ConclusionThe patented"cigarette core"product is good in consistency.In hypospadias repair,it has a high survival rate of mucosal transplantation and a low
