摘 要:目的:探索健身气功养肺方对稳定期慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者认知功能的影响及机制。方法:于2019年11月—2020年7月招募稳定期COPD患者36例,随机分入对照组与干预组。对照组患者接受常规药物治疗和健康宣教,干预组在对照组基础上进行12周健身气功养肺方锻炼(5天/周,2次/天,35min/次)。干预前后对所有患者认知功能进行评价:蒙特利尔认知评估量表(Montreal cognitive assessment,MoCA)、Rivermead行为记忆测试第2版(Rivermead behavioral memory test-II,RBMT-Ⅱ)量表、连线试验(trail making test,TMT)中文修订版、词语流畅性测试(verbal fluency test,VFT)及听觉事件相关电位P300。结果:与干预前相比,干预组MoCA、RBMT-Ⅱ、TMT-A、TMT-B、VFT-音位流畅性分数均显著性改善(P<0.01),Fz、Cz、Pz振幅均显著性增加(P<0.01;P<0.05;P<0.01);干预后组间比较显示,与对照组相比,干预组MoCA分数明显增高(P<0.05),Fz、Cz、Pz潜伏期明显缩短(P<0.01,P<0.05,P<0.05),然而Fz、Cz、Pz振幅未见显著性差异(P>0.05)。结论:健身气功养肺对稳定期COPD患者的整体认知功能、执行功能及词语流畅性可能有积极影响,且可缩短P300波潜伏期,可能是预防COPD患者认知功能减退的一种有效运动策略。Objective:To explore the effect and mechanism of Pulmonary-based Qigong exercise on cognitive function of patients with stable chronic obstructive pulmonary disease(COPD).Method:From November 2019 to July 2020,36 patients with stable COPD were recruited and randomly divided into control group and intervention group.The patients in the control group received routine drug treatment and health education,while the intervention group received 12 weeks of exercise(5 days/week,2 times/day,35 minutes/time)on the basis of the control group.The cognitive function of all patients was evaluated before and after intervention:Montreal Cognitive Assessment(MoCA),Rivermead Behavioral Memory Test-II(RBMT-II),Trail Making Test(TMT),Verbal Fluency Test(VFT)and auditory event-related potential P300.Result:Compared with before intervention,the score of MoCA,RBMT-II,TMT-A,TMT-B or phonemic fluency in the intervention group was significantly improved(P<0.01),and the amplitude of Fz,Cz or Pz was significantly increased(P<0.01;P<0.05;P<0.01);Compared with the control group,the MoCA score of the intervention group was significantly higher(P<0.05),and the latency of Fz,Cz or Pz was significantly shorter(P<0.01,P<0.05,P<0.05),but the amplitude of Fz,Cz or Pz had no statistical difference(P>0.05).Conclusion:Pulmonary-based Qigong exercise may have a positive impact on the global cognitive function,executive function and verbal fluency of patients with stable COPD,and may shorten the P300 wave latency,which may be an effective exercise strategy to prevent cognitive decline of patients with COPD.
