家属参与的自我管理模式在肥胖型PCOS患者IVF-ET助孕中的应用

摘　　要：目的 探讨家属参与的自我管理模式在肥胖型多囊卵巢综合征患者体外受精-胚胎移植术助孕中的应用效果。方法 将拟行体外受精-胚胎移植术助孕的肥胖型多囊卵巢综合征患者100例作为研究对象,按随机原则分为观察组(n=50)和对照组(n=50)。对照组患者实施常规护理模式,观察组在对照组基础上实施家属参与的患者自我管理模式。比较两组患者的婚姻质量、妊娠结局、促排卵时间及实验室指标。结果 观察组患者婚姻质量问卷评分高于对照组,差异有统计学意义(P<0.01)。观察组促性腺激素总用量低于对照组,累积妊娠率(84.78%)高于对照组累积妊娠率(65.85%),差异有统计学意义(P<0.05);观察组患者新鲜周期临床妊娠率(67.39%)高于对照组(60.97%),早期流产率(12.90%)低于对照组(20.00%),但差异均无统计学意义(P>0.05)。结论 家庭参与式的自我管理对肥胖型多囊卵巢综合征患者行体外受精-胚胎移植术结局具有重要价值,可降低促排卵药物用量,提高累积妊娠率。Objective To explore the application effect of self-management model with family participation in the IVF-ET(in vitro fertilization-embryo transplantation)for obese patients with polycystic ovary syndrome(PCOS).Methods One hundred obese PCOS patients who intended to undergo IVF-ET hospitalized in the reproductive center of our hospital from Mar.2019 to Mar.2020 were randomly divided into observation(n=50)and control group(n=50).Control group received routine nursing mode,the observation was plus family-involved self-management mode on the basis of control group.The marital quality,pregnancy outcome,ovulation induction time and laboratory indexes were compared between two groups.Results Scores on the Olson Enrich Marital Inventory were higher in observation than control group,difference was statistically significant(P<0.01);total gonadotropins consumptions were lower and cumulative pregnancy rates higher in observation than control group(84.78%vs.65.85%),the differences were statistically significant(P<0.05);fresh cycle clinical pregnancy rates were higher and early abortion lower in observation than control group(67.39%vs.60.97%,12.90%vs.20.0%),but differences weren't statistically significant(P>0.05).Conclusion Family-involved self-management is of great value to the outcome of IVF-ET for obese PCOS patients,could reduce drug dosage,and increase the cumulative pregnancy rate.
