跗骨窦入路配合撑开器辅助下复位锁定解剖钢板内固定治疗跟骨骨折的疗效观察

摘　　要：目的:探讨跗骨窦入路配合外侧撑开器辅助复位锁定解剖钢板内固定治疗跟骨骨折的临床疗效。方法:选取2019年1月—2021年6月广西南宁市邕宁区中医医院收治的42例关节内跟骨骨折患者为观察对象。所有患者采用跗骨窦入路配合外侧撑开器辅助下复位锁定解剖钢板内固定治疗。观察并记录患者骨折愈合情况、手术相关情况、疼痛和术后并发症情况。结果:术后患者跟骨宽度、Gissane角低于术前,跟骨高度、Bolher角高于术前,差异有统计学意义(P=0.001);患者手术时间(71.45±6.52)min,视觉模拟评分法评分(2.80±1.16)分,出现皮肤延迟愈合1例,创伤性关节炎1例,并发症发生率为4.76%;患者术后治疗优良率为90.48%。结论:跗骨窦入路配合外侧撑开器辅助复位锁定解剖钢板内固定治疗跟骨骨折的临床疗效显著,具有操作简便、手术时间短、复位效果好、创伤小、并发症少、功能恢复快等优点。Objective:To investigate the clinical effect of tarsal sinus approach combined with lateral spinner assisted reduction and locking anatomical plate internal fixation in the treatment of calcaneal fracture.Methods:Forty-two patients with intraarticular calcaneal fracture admitted to Yongning District Hospital of Traditional Chinese Medicine from January 2019 to June 2021 were selected as the observation objects.All patients were treated by tarsal sinus approach with lateral spatula assisted reduction and locking anatomical plate internal fixation.Fracture healing,surgery related conditions,pain and postoperative complications were observed and recorded.Results:Calcaneal bone width and Gissane angle were lower after operation,while calcaneal bone height and Bolher angle were higher than before operation,with statistical significance(P=0.001).The operative time of the patient was(71.45±6.52)min,and the visual analogue score was(2.80±1.16).Delayed skin healing occurred in 1 case,traumatic arthritis occurred in 1 case,and the complication rate was 4.76%.The excellent rate of postoperative treatment was 90.48%.Conclusion:The treatment of calcaneal fracture with tarsal sinus approach combined with lateral spatula assisted reduction and locking anatomical plate internal fixation has significant clinical effect,and has the advantages of simple operation,short operation time,good reduction effect,less trauma,fewer complications,and fast functional recovery.
