左旋多巴联合司来吉兰或普拉克索治疗帕金森病的药物剂量多中心研究

摘　　要：目的:分析左旋多巴联合司来吉兰或普拉克索治疗帕金森病的真实世界临床给药剂量情况,为合理用药提供参考。方法:基于全国30个三甲医院随机抽取的2014-2019年的帕金森患者用药真实世界处方数据,筛选帕金森病治疗药物,根据实际用药情况分为单用左旋多巴、左旋多巴+司来吉兰、左旋多巴+普拉克索3组的处方数据,分析不同方案中左旋多巴的给药剂量,以及小剂量左旋多巴(≤400.00 mg·d^(-1))处方占比。结果:左旋多巴+司来吉兰组的左旋多巴等效日剂量与左旋多巴+普拉克索组(P<0.01)、左旋多巴组(P<0.01)差异均有统计学意义,左旋多巴+普拉克索组与左旋多巴组的左旋多巴等效日剂量差异并无统计学意义(P=0.76)。不同年龄段、年份和地区的左旋多巴等效日剂量有一定差异,小剂量左旋多巴处方占比呈逐年增加。结论:帕金森患者联合使用司来吉兰时可能需要减少左旋多巴用药剂量,而普拉克索的联合使用不需要调整左旋多巴的应用剂量。OBJECTIVE To analyze the actual clinical dosage of levodopa combined with selegiline or pramipexole in the treatment of Parkinson’s disease(PD),so as to provide references for rational drug utilization.METHODS Based on the real-world prescription data of PD patients from 2014 to 2019 were randomly selected from 30 tertiary hospitals across the country.And the prescription data of levodopa alone,levodopa plus selegiline,and levodopa plus pramipexole were selected to analyze the dose of levodopa administered by different schemes and the proportion of prescriptions for low-dose levodopa(≤400.00 mg·d-1).RESULTS The equivalent daily dose of levodopa in the levodopa plus selegiline group was significantly different from that in the levodopa plus pramipexole group(P<0.01)and the levodopa group(P<0.01).There was no significant difference in the equivalent daily dose of levodopa between the levodopa plus pramipexole group and the levodopa group(P=0.76).There were certain differences in the equivalent daily dose of levodopa in different age groups,periods,and regions.The proportion of low-dose levodopa prescriptions was increasing year by year.CONCLUSION The combined utilization of selegiline and levodopa in PD patients may need to reduce the dose of levodopa,while the combined use of pramipexole and levodopa does not require adjustment of the dose of levodopa.
