住培医师参与二级剖宫产手术的安全性分析

摘　　要：目的 探讨住培医师作为一助参与二级子宫下段横切口剖宫产手术的安全性。方法 回顾性选择2017年7月—2018年7月漯河市中心医院166例二级子宫下段横切口剖宫产手术,其中由2015—2016级的7名住培医师担任一助的手术作为观察组(103例),由本院住院医师(已完成住培)及以上资质的医生作为一助进行的手术作为对照组(63例)。比较两组患者手术相关指标及术后并发症情况。结果 观察组和对照组之间的手术时长、术中出血量、术后住院时间、术后肛门排气时间、术前术后HGB差值和HCT差值对比,差异均无统计学意义(P>0.05)。术后并发症(切口脂肪液化、产后出血、产褥病率、子宫瘢痕憩室)发生率对比,差异无统计学意义(P>0.05)。结论 经过培训且考核合格的住培医师担任二级剖宫产手术一助是安全的。Objective To explore the safety of resident doctors under standardized residency training perform the grade 2 transverse incision lower uterine cesarean section as first assistant.Methods Among 166 grade 2 transverse incision lower uterine cesarean sections from Jul.2017-Jul.2018,103 surgeries that 7 resident doctors under standardized residency training(Grade 2015-2016)performed as first assistant were selected as observation group,and 63 surgeries that resident doctors(completed residential training)or above as first assistant were selected as control group,then the surgery-related indicators and postoperative complications between 2 groups were compared.Results The differences were not statistically significant(P>0.05)between the 2 groups on surgery duration,intraoperative bleeding volume,hospital stay,anal exhaust time after surgery,HCB and HCT differences before and after surgery.The difference was also not statistically significant(P>0.05)on postoperative complications incidence rate,including incision fat liquefaction,postpartum hemorrhage,puerperal morbidity,cesarean scar diverticulum.Conclusion It's safe to have resident doctors under standardized residency training and passed exams to be the first assistant of the grade 2 transverse incision lower uterine cesarean section.
