摘　　要：目的:探讨应用双翼推进皮瓣成形指蹼治疗先天性并指畸形的临床疗效。方法:自2014年8月至2017年8月我院对30例先天性并指畸形患者,在并指根部背侧设计凸V形尖端、蒂部两侧带三角形皮肤的双翼皮瓣与掌侧锚形切口对应缝合,成形指蹼同时关闭指根部创面。并连的远端指体采用锯齿状皮瓣分离,侧方创面减脂后缝合,指侧方较紧创面不能完全缝合者移植全厚皮片修复。术后给予抗生素预防感染,拆线后指导患者进行功能锻炼。结果:所有病例指蹼及指根部均未植皮,11例指侧方所植全厚皮片均成活,创面均一期愈合。术后随访6-12个月,屈伸功能良好,指蹼深度、宽度正常。结论:应用双翼推进皮瓣成形指蹼治疗先天性并指畸形具有良好的临床效果,是治疗先天性并指畸形的良好方式之一。Objective:To explore the clinical effect of the treatment of congenital syndactyly by using the double wing advancement flap to form the finger web.Methods:From August 2014 to August 2017,30 cases of congenital syndactyly were treated in our hospital.The flap with v-shaped tip and triangular skin on both sides of the pedicle was designed on the dorsal side of the combined finger root and sutured with the volar anchor incision to form the web and close the wound on the root of the finger.The connected distal phalanges were separated with a serrated skin flap,lateral wound was sutured after reducing fat,and Full-thickness skin graft was used to repair the wound which could not be sutured completely.After operation,antibiotics were given to prevent infection,and functional exercises were conducted after the stitches were removed.Results:No skin grafing was performed on the web and root of fingers in all cases,all the 1l cases of full-thickness skin graft on the side of finger survived and the wound healed in one stage.After 6-12 months follow-up,good flexion and extension function obtained,web depth and width were normal.Conclusion:The treatment of congenital syndactyly with finger web by double wing advancement flap has a good clinical effect,which is one of the best ways to treat congenital syndactyly.
