下肢轴向牵引器联合闭合复位钢板内固定对股骨远端骨折的疗效观察

摘　　要：目的观察下肢轴向牵引器联合闭合复位钢板内固定对股骨远端骨折患者的疗效。方法选取2021年1月至2022年1月湖州市第一人民医院收治的股骨远端骨折患者46例为研究对象,按照住院号的奇偶数字分为对照组(奇数,给予闭合复位钢板内固定)23例和联合组(偶数,给予下肢轴向牵引器联合闭合复位钢板内固定)23例。比较两组患者手术相关指标、髋关节功能、临床疗效及并发症发生情况。结果联合组患者术中出血量、术中透视次数、手术时间及骨痂形成时间均低于对照组(均P<0.05);与术前比较,两组患者术后3、6个月Harris评分均升高(均P<0.05),且联合组患者术后3、6个月Harris评分均高于对照组(均P<0.05);两组患者临床疗效及并发症发生率比较差异均无统计学意义(均P>0.05)。结论股骨远端骨折患者采用下肢轴向牵引器联合闭合复位钢板内固定可有效改善髋关节功能,减少术中透视次数及出血量,促进恢复,且操作方便,安全有效。Objective To observe the efficacy of lower extremity axial distractor combined with closed reduction and plate internal fixation for treatment of distal femoral fracture.Methods Forty-six patients with distal femoral fracture admitted to Huzhou First People's Hospital from January 2021 to January 2022 were randomly divided into two groups with 23 cases in each group.Patients in control group received closed reduction and plate internal fixation and those in study group received lower extremity axial distractor combined with closed reduction and plate internal fixation.The operative indexes,hip function,clinical efficacy and complications were compared between the two groups.Results The intraoperative blood loss,intraoperative fluoroscopy times,operation time,and callus formation time in the study group were significantly lower than those in the control group(all P<0.05).The Harris scores at 3 months and 6 months were significantly increased after operation in both groups(all P<0.05),meanwhile,the Harris scores of the study group were significantly higher than those of the control group(both P<0.05).There were no significant differences in clinical efficacy and complication rate between two groups(both P>0.05).Conclusion Lower extremity axial distractor combined with closed reduction and plate internal fixation for patients with distal femoral fracture can effectively enhance hip joint function,reduce the number of intraoperative fluoroscopy with easy operation and safely.
