社会组织促进HIV感染者和艾滋病患者随访关怀效果评估

摘　　要：目的 评估社会组织参与艾滋病防治基金(China AIDS Fund for Non-governmental Organizations,CAFNGO)支持的社会组织促进我国人类免疫缺陷病毒(human immunodeficiency virus, HIV)感染者/艾滋病患者的随访质量提升的成效,为提高社会组织服务质量和基金项目效果提供参考。方法 通过CAFNGO项目管理信息系统收集2016—2022年HIV感染者/艾滋病患者随访管理和关怀救助项目的相关数据,从经费投入、核心指标产出进行整理分析,结合社会组织调研访谈进行信息补充。结果 2016—2022年CAFNGO支持2 066个随访管理和关怀救助项目,资助经费9 898万元,其中38.96%(805/2 066)项目由民政登记的社会组织独立执行,其他项目均由培育基地联合未民政登记的社会组织共同执行。社会组织共提供了171.44万人次随访关怀服务,为44.65万例HIV感染者和艾滋病患者提供了至少每年2次面对面服务。民政登记社会组织和培育基地联合未登记社会组织的项目动员阴性配偶/性伴HIV检测率分别为92.55%和94.00%,抗病毒治疗(antiretroviral therapy,ART)比例分别为97.54%和96.37%,成功率分别为90.22%和90.49%,三个指标差异均有统计学意义(χ^(2)=86.764、490.777、7.104,P<0.01);不同类型培育基地项目之间3个指标存差异有统计学意义(χ^(2)=14.657、2 279.864、1 312.936,P<0.01)。结论 CAFNGO支持的社会组织在促进HIV感染者/艾滋病患者随访管理和关怀救助质量发挥了有效的补充作用。Objective To assess the role of community-based organizations(CBOs)supported by China AIDS Fund for Non-governmental Organizations(CAFNGO) in promoting the improvement of follow-up for people living with human immunodeficiency virus(HIV)/acquired immunodeficiency syndrome(AIDS)(PLWH) patients in China,so as to provide evidence for elevating the quality of services provided by CBOs and improving the effect of CAFNGO projects. Methods The data related to HIV/AIDS patients follow-up and care support projects in the period from 2016 to 2022 were extracted from CAFNGO's project information management system,and analyzed in the aspects of funding input, core indicator outcomes. Additional information were collected through interviews with CBOs. Results From 2016 to 2022, CAFNGO supported 2 066 follow-up and care projects for PLWH, providing a total funding of 98. 98 million yuan. Of all the supported projects,38. 96%(805/2 066) were independently implemented by the registered CBOs, and the others were jointly implemented by incubation bases and unregistered CBOs. Within those projects, a total of 1. 714 4 million person-times of follow-up and care services were provided, and face-to-face services at least twice a year for 0. 446 5 million HIV/AIDS patients. In the projects implemented by registered CBOs and those by incubation bases in collaboration with unregistered CBOs, the HIV detection rates of HIV-negative spouses/sexual partners were 92. 55% and 94. 00%, respectively;the proportions of antiretroviral therapy(ART) were 97. 54% and 96. 37%, respectively;and the treatment success rates were 90. 22% and 90. 49%, respectively;all with statistically significant difference(χ^(2)=86. 764, 490. 777, 7. 104, all P<0. 01).
