不同类型培育基地促进社会组织重点人群艾滋病防治干预效果研究

摘　　要：目的 分析国家社会组织参与艾滋病防治基金(China AIDS Fund for Non-govermental Organizations,CAFNGO)重点人群干预领域中不同类型培育基地促进社会组织工作的成效,总结经验和建议。方法 整理2016—2022年CAFNGO项目管理信息系统中培育基地联合未民政登记的社会组织重点人群干预项目数据,采用χ^(2)检验分析不同类型培育基地数量变化及项目检测、阳性发现、转介治疗等指标产出差异。结果 2016—2021年CAFNGO共批准了6 136个项目,培育基地联合未登记的社会组织承担项目占项目总数的61.98%(3 803/6 136);其中批准了重点人群干预项目4 072个,由培育基地联合未登记的社会组织承担了的项目占重点人群干预项目总数的62.18%(2 532/4 072),覆盖全国31个省(市区)。CAFNGO重点人群干预项目中培育基地以疾病预防控制中心(Center for Disease Control and Prevention,CDC)为主,但其机构数、项目数和经费量逐年下降。社会组织类培育基地与CDC类培育基地呈现完全相反趋势,机构数从2016年的5家上升至2021—2022年的23家。学/协会和医疗机构类培育基地2021—2022年参与机构数量较2016年分别上升36.00%和50.00%。2016—2022年社会组织类培育基地重点人群干预项目的干预检测转化率(25.26%)和阳性转介治疗率(85.97%)最高,学/协会类培育基地项目初筛阳性率(4.02%)和确证阳性率(3.18%)最高。四类培育基地干预项目的干预检测转化率、初筛阳性率、确证检测率、确证阳性率及确证阳性转介治疗率差异有统计学意义(χ^(2)=12 126.024 6、 1 285.815 9、262.954 2、924.908 7、225.583 7,P<0.001)。结论 社会组织和学/协会类培育基地力量不断发展壮大,但目前仍数量较少,应进一步完善CAFNGO培育基地机制建设,发挥社会组织类培育基地及专业机构(CDC和医疗机构)的各自优势,形成合力,促进未登记的社会组织参与艾滋病防治工作。Objective To analyze the effectiveness of different types of incubation bases of China AIDS Fund for Non-govermental Organizations(CAFNGO) in the filed of key population intervention in promoting the work of community-based organizations(CBOs), and summarize experience and suggestions. Methods Project data from 2016—2022 were extracted from the CAFNGO project management information system and analyzed. The change of the number of incubation bases in the filed of key population intervention in China were compared across different types of bases. The output indicators such as the conversion rate of intervention test, the positive rate of initial screening, the rate of confirmatory test, the positive rate of confirmatory test, and the rate of referral to treatment for HIV-positive cases were analyzed by χ^(2) test. Results From 2016 to 2021, 6 136 projects were approved by CAFNGO, of which 61. 98%(3 803/6 136) were jointly undertaken by incubation bases and CBOs. 4 072 out of 6 136 intervention projects for key population were approved, and 62. 18%(2 532/4 072)projects in 31 provinces were undertaken by incubation bases with CBOs. The participating incubation bases were mainly centers for disease control and prevention(CDC), but the number of participating CDCs, the number of projects, and the amount of funds decreased year by year. In contrast, incubation bases of non-governmental organization(NGO) showed opposite trend. The number increased from 5 bases in 2016 to 23 bases in 2021—2022. From 2016 to 2022, the number of incubation bases of the type of academic associations and hospitals increased by 36. 00% and 50. 00%, respectively. 2016—2022, the conversion rate of intervention test from intervention(25. 26%) and the referral rate of Antiviral therapy for HIV-positive(85. 97%) of high risky population intervention projects implemented by NGO incubation bases were highest, The positive rate of initial screening(4. 02%) and the positive rate of confirmatory test(3. 18%) of high risky population intervention
