苍耳子散加减联合羧甲司坦口服溶液治疗小儿鼻渊临床疗效及对血清IFN-γ、IL-8水平的影响

摘　　要：目的:观察苍耳子散加减联合羧甲司坦口服溶液治疗小儿鼻渊的临床疗效及对血清干扰素-γ(IFN-γ)、白细胞介素-8(IL-8)水平的影响。方法:选择鼻渊患儿80例,按随机数字表法分为联合组和对照组各40例。对照组给予羧甲司坦口服溶液,联合组在对照组的基础上给予苍耳子散加减口服。2组均治疗2周。比较临床疗效及不良反应情况,主要症状视觉模拟评分法(VAS)评分、鼻窦CT扫描Fund-Mackay评分、鼻内镜Lund-Kennedy评分,血清IFN-γ、IL-8表达水平。结果:联合组总有效率为95.00%,对照组为75.00%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组鼻塞、头疼、打喷嚏、流涕等主要症状VAS评分均较治疗前降低(P<0.05),且联合组上述各项VAS评分均低于对照组(P<0.05)。治疗后,2组FundMackay、Lund-Kennedy评分均较治疗前降低(P<0.05),且联合组上述2项评分均低于对照组(P<0.05)。治疗后,2组血清IFN-γ水平较治疗前升高(P<0.05),IL-8水平较治疗前降低(P<0.05);且联合组IFN-γ水平高于对照组(P<0.05),IL-8水平低于对照组(P<0.05)。治疗期间,联合组不良反应发生率为10.00%,对照组为12.50%,2组比较,差异无统计学意义(P>0.05)。结论:苍耳子散加减联合羧甲司坦口服溶液治疗小儿鼻渊可显著改善临床症状体征,有效控制机体炎症反应,疗效优于单纯西药治疗。Objective:To observe the clinical effect of modified Cangerzi Powder combined with Carbocisteine Oral Solution on sinusitis and its effect on levels of serum interferon-γ(IFN-γ)and interleukin-8(IL-8)in children.Methods:A total of 80 children with sinusitis were selected and divided into the combination group and the control group according to the random number table method,with 40 cases in each group.The control group was given Carbocisteine Oral Solution for treatment,and the combination group was additionally treated with oral administration of modified Cangerzi Powder in the form of decoction.The two groups were treated for two weeks.The clinical effects,scores of Visual Analogue Scale(VAS)for main symptoms,Fund-Mackay scores for CT scanning of sinusitis,Lund-Kennedy scores for nasal endoscopy,expression levels of serum IFN-γand IL-8 and incidence of adverse reactions were compared between the two groups.Results:The total effective rate was 95.00%in the combination group,and 75.00%in the control group,the difference being significant(P<0.05).After treatment,the VAS scores for main symptoms,including nasal obstruction,headache,sneezing and running nose were decreased when compared with those before treatment(P<0.05),and the above four VAS scores in the combination group were lower than those in the control group(P<0.05).After treatment,the scores of Fund-Mackay and Lund-Kennedy in the two groups were decreased when compared with those before treatment(P<0.05),and the above two scores in the combination group were lower than those in the control group(P<0.05).After treatment,the serum IFN-γlevels in both groups were increased when compared with those before treatment(P<0.05),and the IL-8 levels were decreased when compared with those before treatment(P<0.05);the serum IFN-γlevel in the combination group was higher than that in the control group(P<0.05),and the IL-8 level was lower than that in the control group(P<0.05).During treatment,the incidence of adverse reactions was 10.00%and 12.5%in the control gr
