从六经辨治盗汗思路探析

摘　　要：通过梳理汗证的源流,阐明盗汗的病名由来,并将历代医家有关盗汗的病机进行概括与述评,明晰盗汗的病机涉及阴虚、血虚、阳虚、阴阳两虚等虚证,也涉及湿、热、瘀等实证,指出“自汗属阳虚,盗汗属阴虚”这一观点存在时弊;进而探讨从六经辨治盗汗的临证思路,并分别从“六经-营卫-玄府”病机链、六经与营卫的关系、从六经辨治盗汗的治疗概况等方面进行了论述,认为盗汗辨治从常规的八纲辨证、脏腑辨证、病因辨证等思路均难以统领全部,但从六经辨治则能融合八纲与脏腑、统摄营卫与气血,却是一种多维立体的辨治思路,适合盗汗这种伴见于内科杂病中的常见病证的诊治。By sorting out the origin and flow of sweat syndrome,this article clarifies the origin of night sweat,and summarizes and reviews the pathogenesis of night sweat by Chinese medicine practitioners of all dynasties.It is clear that the pathogenesis of night sweat involves yin deficiency,blood deficiency,yang deficiency,yin-yang deficiency and other deficiency syndromes,as well as the excess syndromes like dampness,heat and stasis.It is pointed out that the idea that“spontaneous sweating is ascribed to yang deficiency,and night sweat is ascribed to yin deficiency"has disadvantages of the times.The paper further discusses the clinical thinking of treating night sweat from six-channel syndrome differentiation,and discusses the pathogenesis chain of"six channels-nutritious qi and defensive qi-sweat pore”,the relationship between the six channels and nutritious qi and defensive qi,and the treatment overview of night sweat from six-channel syndrome differentiation.It is believed that it is difficult to completely treat night sweat from the conventional treatment thoughts of eight-principle syndrome differentiation,zang-fu syndrome differentiation and syndrome differentiation based on etiology.However,if six-channel syndrome differentiation is adopted,it can integrate the eight principles with zang-fu and control nutritious qi,defensive qi,qi and blood,which is a multidimensional and three-dimensional treatment idea,suitable for the diagnosis and treatment of night sweat,a common disease associated with miscellaneous diseases in internal medicine.
