摘　　要：目的对比三孔法腹腔镜胆囊切除手术过程中,使用5 mm Trocar或10 mm Trocar作为主操作孔切除胆囊时,两种方式手术应用上的优势和不足,从而提高患者的治疗质量。方法收集十堰市太和医院肝胆胰外科诊疗中心2021年6月至2022年6月期间,由同一诊疗组收治的胆囊良性疾病患者共100例,其中50例使用5 mm Trocar作为主操作孔行腹腔镜胆囊切除术,设为实验组;50例使用10 mm Trocar作为主操作孔,设为对照组。分别对比实验组和对照组手术时间、术中出血量、术后胃肠道恢复情况、术后疼痛情况、术后住院时间、住院费用的差异,并进行统计学分析。结果两种腹腔镜胆囊切除术均可顺利完成。患者手术时间、术中出血量、术后胃肠道恢复情况、术后住院时间、住院费用比较,差异均无统计学意义(P>0.05)。实验组患者主操作孔伤口术后疼痛程度相较于对照组较低,且切口较小利于美容缝合,术后伤口更美观。结论三孔法腹腔镜胆囊切除手术过程中,使用5 mm Trocar作为主操作孔优于10 mm Trocar作为主操作孔,手术质量更高,有益于患者,此种术式值得推广。【Objective】To Compare the advantages and disadvantages of the surgical application of the two methods when using 5mm Trocar or 10mm Trocar as the main operating hole during the three-well laparoscopic cholecystectomy procedure,resulting in the improvement of the treatment quality of patients.【Methods】From June 2021 to June 2022,100 patients with benign gallbladder diseases were admitted in the same diagnosis and treatment group,of which 50 cases used 5mm Trocar as the main operating hole for laparoscopic cholecystectomy,set as the experimental group;50 cases used 10mm Trocar as the main operating hole,set as the control group.The differences in operation time,intraoperative bleeding volume,postoperative gastrointestinal recovery,postoperative pain,postoperative hospital stay,and hospital cost were compared between the experimental group and the control group,and statistical analysis was conducted.【Results】Both types of laparoscopic cholecystectomy could be performed successfully.There were no significant differences in operative time,intraoperative bleeding,postoperative gastrointestinal recovery,postoperative hospital stay,or hospitalization costs.The degree of the postoperative pain of the main operating hole wound was lower than that of the control group,and the incision was less conducive to cosmetic suture,and the postoperative wound was more beautiful.【Conclusion】During three-hole laparoscopic cholecystectomy,using 5mm Trocar as the main operating hole is better than 10mm Trocar as the main operating hole.The operation quality is higher and beneficial to patients.This operation is worth promoting.
