摘 要:目的在2组不同水平血清总免疫球蛋白E(IgE)的特应性皮炎(AD)患儿中,分析食物特异性IgE(sIgE)的阳性率和类别特点。方法选择2018年1月3日—2023年1月10日来自本院就诊AD患儿170例作为研究对象,依据血清总IgE的水平将患儿分为低IgE(<200 IU/ml)AD组和高IgE(≥200 IU/ml)AD组,同期选择健康儿童50例作为对照组。采用免疫比浊法检测血清总IgE水平、荧光免疫法检测血清食物(鸡蛋白、牛奶、小麦、花生、大豆、虾)sIgE水平。结果与对照组比较,AD组、低IgE AD组和高IgE AD组食物sIgE阳性率均升高(P<0.05);与低IgE AD组比较,高IgE AD患儿食物sIgE的阳性率升高(P<0.05)。在低IgE和高IgE AD患儿中,0岁~3岁年龄组食物sIgE阳性率占比均为最高。低IgE AD组食物sIgE的阳性率从高到低依次为鸡蛋白(20.63%)、牛奶(19.05%)、小麦(8.73%)、花生(3.97%)、虾(2.38%)、大豆(2.38%);高IgE AD组食物sIgE的阳性率从高到低依次为牛奶(45.45%)、鸡蛋白(36.36%)、小麦(36.36%)、花生(29.55%)、大豆(20.45%)、虾(4.55%)。低IgE AD患儿中,1种过敏原阳性者占16.67%,2种及以上阳性者占15.87%。高IgE AD患儿1种过敏原阳性者占18.18%,2种及以上阳性者占59.09%。结论0岁~3岁AD儿童最易出现食物过敏,高IgE AD患儿存在多种食物sIgE升高的特点。Objective This paper aims to analyze the positive rate and classification of food-specific IgE(sIgE)in two groups of children with atopic dermatitis(AD)with different levels of total immunoglobulin E(IgE).Methods A total of 170 chil⁃dren with AD admitted to our hospital from January 3,2018 to January 10,2023 were selected as subjects.According to the level of total serum IgE,the children were divided into low IgE(<200 IU/ml)AD group and high IgE(≥200 IU/ml)AD group,and 50 healthy children were selected as control group.Serum total IgE was detected by immunoturbidimetry and serum sIgE of food(egg white,milk,wheat,peanut,soybean,shrimp)were detected by fluorescence immunoassay.Results Com⁃pared with control group,the positive rate of sIgE in AD group,low IgE AD group and high IgE AD group were increased(P<0.05).The positive rate of food sIgE in high IgE AD group was significantly higher than low IgE AD group(P<0.05).In children with low IgE and high IgE AD,the positive rate of food sIgE was the highest in 0-3 years old age group.The positive rates of sIgE in low IgE AD group showed a decreasing trend in egg white(20.63%),milk(19.05%),wheat(8.73%),peanut(3.97%),shrimp(2.38%)and soybean(2.38%).The positive rates of sIgE in high IgE AD group were observed in milk(45.45%),egg white(36.36%),wheat(36.36%),peanut(29.55%),soybean(20.45%)and shrimp(4.55%).In children with low IgE AD,16.67%were positive for one allergen,and 15.87%for two or more.In children with high IgE AD,18.18%were positive for one allergen,and 59.09%for two or more allergen.Conclusion Children aged 0-3 years with AD are most prone to food allergy.Children with high IgE AD have the characteristics of increased sIgE of various foods.
