卵巢硬化性间质瘤伴腹水、糖类抗原125升高1例报告并文献复习

摘　　要：卵巢硬化性间质瘤是一种卵巢良性肿瘤,多见于年轻女性,因发病率低,临床症状无特异性,超声、核磁等辅助检查、术中冰冻病理诊断困难,常造成过度治疗。本文对中山大学附属第八医院2022年7月14日收治的1例卵巢硬化性间质瘤伴腹水、糖类抗原125升高患者进行分析。患者因月经紊乱于当地医院就诊时发现右侧卵巢包块,复查超声见卵巢包块进行性增大,同时伴腹腔积液,患者入院完善相关检查后行剖腹探查术,术中行右侧卵巢囊肿剔除术,最终诊断为右侧卵巢硬化性间质瘤。术后复查超声见盆腔无异常包块,腹腔积液消失。术后随访9个月,月经均正常,未复发。本文对该病例临床表现、诊断、治疗及预后进行分析,同时总结经验,并进行文献学习。Sclerosing stromal tumor of the ovary is a kind of benign tumor of ovary,mostly seen in young women,which often results in overtreatment due to its low incidence,non-specific clinical symptoms,difficulties in adjuvant examinations such as ultrasound and NMR,and intraoperative frozen pathology diagnosis.In this paper,a patient with sclerosing stromal tumor of the ovary accompanied by ascites and elevated carbohydrate antigen 125 admitted to the Eighth Affiliated Hospital of Sun Yat-sen University on July 14,2022 was analyzed.The patient was found to have a right ovarian mass at the local hospital due to menstrual disorder.Reexamination ultrasound showed progressive enlargement of the solid ovarian mass accompanied by abdominal fluid.Exploratory laparotomy was performed after relevant examinations were completed.Intraoperative excision of the right ovarian cyst was performed,and the final diagnosis was right sclerosing stromal tumor of the ovary.Postoperative ultrasonography showed no abnormal mass in the pelvic cavity and no peritoneal effusion.After 9 months of follow-up,menstruation was normal without recurrence.In this paper,the clinical manifestations,diagnosis,treatment and prognosis of this case were analyzed.At the same time,experience was summarized and literature study was carried out.
