创面磨削痂术联合外用rhGM-CSF对Ⅱ度烧伤患儿创面愈合的影响

摘　　要：目的:分析创面磨削痂术联合外用重组人粒细胞-巨噬细胞集落刺激因子(Recombinant human granulocyte macrophage colony-stimulating factor,rhGM-CSF)对Ⅱ度烧伤患儿创面愈合的影响。方法:选取2019年5月-2022年5月笔者医院收治的116例Ⅱ度烧伤患儿为研究对象,随机分为对照组与研究组各58例,对照组患儿采用创面磨削痂术治疗,研究组患儿采用创面磨削痂术联合外用rhGM-CSF治疗。比较两组患儿的临床疗效、创缘反应、创面渗出量、创面溶痂率与溶痂时间、创面愈合率与愈合时间、住院时间及安全性。结果:治疗4周后,研究组患儿临床有效率高于对照组,创缘反应及创面渗出量评分分布优于对照组,差异均有统计学意义(P<0.05);治疗2周及4周后,研究组患儿创面溶痂率高于对照组,创面完全溶痂时间短于对照组,创面愈合率高于对照组,创面完全愈合时间短于对照组,平均住院时间短于对照组,差异均有统计学意义(P<0.05);两组患儿不良反应发生率比较差异无统计学意义(P>0.05)。结论:创面磨削痂术联合外用rhGM-CSF治疗Ⅱ度烧伤患儿疗效较好,能有效改善患儿创面的创缘反应与渗出量,促进创面溶痂与愈合,缩短住院时间,安全性较好。Objective To analyze the effects of wound dermabrasion combined with recombinant human granulocyte-macrophage colony-stimulating factor(rhGM-CSF)on wound healing in children with degree-Ⅱ burns.Methods A total of 116 children with second-degree burn treated in our hospital from May 2019 to May 2022 were selected as the study objects,and were randomly divided into the control group and the study group,with 58 cases in each group.The control group was treated with wound grinding callus,and the study group was treated with wound grinding callus combined with external rhGM-CSF.The clinical efficacy,wound edge response,wound exudation,wound dissolution rate and time,wound healing rate and healing time,hospital stay and safety were compared between the two groups.Results After 4 weeks of treatment,the clinical effective rate of the study group was higher than that of the control group,and the score distribution of the wound edge response and wound exudation was better than that of the control group,with statistical signifi cance(P<0.05).After 2 and 4 weeks of treatment,the rate of wound scab dissolution in the study group was higher than that in the control group,the time for complete scab dissolution was shorter than that in the control group,the rate of wound healing was higher than that in the control group,the time for complete wound healing was shorter than that in the control group,and the average length of hospital stay was shorter,with statistical signifi cance(P<0.05).There was no signifi cant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Curative effect of wound dermabrasion combined with rhGM-CSF is good on children with degree-II burns,which can eff ectively improve wound edge response and exudation volume,promote wound scab dissolution and healing,and shorten hospitalization time,with good safety.
