超脉冲CO_(2)激光联合5-氨基酮戊酸光动力治疗中重度痤疮的临床疗效评价

摘　　要：目的:分析超脉冲CO_(2)激光联合5-氨基酮戊酸(5-ALA)光动力治疗中重度痤疮的临床疗效。方法:选取2019年8月-2021年7月于笔者医院接受治疗的脓疱型或结节囊肿型痤疮患者68例,根据患者就诊的先后顺序,单号患者纳入观察组,给予超脉冲CO_(2)激光与5-ALA光动力联合治疗,双号患者纳入对照组,给予5-ALA光动力治疗,各34例。比较两组间的临床疗效及不良反应情况。结果:治疗前两组间粉刺、丘疹、脓疱、结节囊肿数目及痤疮综合分级(Globalacnegrading system,GAGS)评分相比差异无统计学意义(P>0.05),治疗后两组各类型皮损数目及GAGS评分均显著下降,与对照组相比,观察组皮损数目更少、GAGS评分更低、治疗有效率更高(88.24%VS67.65%),差异均有统计学意义(P<0.05)。观察组患者不良反应发生率(23.53%)与对照组(17.65%)相比差异无统计学意义(P>0.05)。结论:超脉冲CO_(2)激光与5-ALA光动力联合治疗中重度痤疮患者临床疗效较好,且未增加不良反应发生率,值得临床推广应用。Objective To analyze the clinical efficacy of ultra-pulsed CO_(2) laser combined with 5-aminolevulinic acid(5-ALA)photodynamic therapy in patients with moderate to severe acne.Methods A total of sixty eight patients with pustular or nodulocystic acne who received treatment in the author's hospital from August 2019 to July 2021 were prospectively enrolled.According to the order of enrollment,34 patients with odd numbers were included in the observation group and treated with ultra-pulsed CO2 laser and 5-ALA photodynamic combination therapy,and 34 patients with even numbers were included in the control group and treated with 5-ALA photodynamic therapy.The clinical efficacy and adverse reactions were compared in both groups.Results Before treatment,the numbers of acne,papules,pustules,nodular cysts and GAGS score were not signifi cantly different in both groups(P>0.05).After treatment,the numbers of each type of skin lesions and GAGS score decreased significantly,compared with those in the control group,fewer skin lesions,lower GAGS score,and higher treatment efficiency occurred in the observation group(88.24% VS 67.65%),and the above-mentioned differences were statistically signifi cant(P<0.05).There was no significant difference in the incidence of adverse reactions in the observation group compared with that in the control group(23.53%VS 17.65%),(P>0.05).Conclusion Ultra-pulsed CO_(2) laser and 5-ALA photodynamic combination therapy has good clinical effi cacy in patients with moderate to severe acne,without increasing the incidence of adverse reactions.It is worthy of being widely popularized in the clinical application.
